Description
Neomycin sulfate is a broad-spectrum antibiotic. Dexamethasone is a topical anti-inflammatory steroid. Acetic acid functions to produce a low pH to assist in the control of bacterial infection.

Pharmacology
Neomycin sulfate is a broad-spectrum antibiotic. Dexamethasone is a topical anti-inflammatory steroid. Acetic acid functions to produce a low pH to assist in the control of bacterial infection.
Mechanism of action
Neomycin kills bacteria; Dexamethasone reduces inflammation; Acetic acid creates low pH to control bacterial growth.
Dosage
Adults and children (≥2 years): One metered dose (60 mg) to be administered directly into each affected ear three times daily. Continue until two days after symptoms have disappeared. Discontinue if no clinical improvement after 7 days.
Administration
Shake well before use. Before first use, press actuator down several times to obtain a fine spray. Do not inhale the spray. Place nozzle tip into ear opening and press once on the actuator.
Missed dose
If you miss a dose, apply it as soon as possible. However, if it is almost time for your next application, skip the missed dose.
Side effects
Common: Transient stinging or burning sensation for first few days of treatment, skin hypersensitivity reactions (irritation, itching, dermatitis).
Other: Blurred vision has been reported with corticosteroid use.
Precautions & warnings
Discontinue if irritation or rash occurs, condition worsens, or no improvement within 7 days. Risk of drug-induced deafness with aminoglycosides in patients with tympanic membrane perforation. Theoretical risk of adrenal suppression in children with prolonged use.
Contraindications
Hypersensitivity; perforated tympanic membrane or tympanostomy tube; children under 2 years of age.

Use in pregnancy
There are no adequate data on the use of dexamethasone in pregnant women. Animal studies have shown topical corticosteroids to cause reproductive toxicity. This ear spray is not recommended during pregnancy.

Use in lactation
No reports describing the use of neomycin or dexamethasone during lactation are available. A decision must be made whether to discontinue breast-feeding or discontinue therapy.
Pediatric use
Not suitable for infants and neonates under 2 years of age.
Geriatric use
No specific dosage adjustment is necessary for elderly patients.
Renal / hepatic impairment
Aminoglycoside toxicity may be enhanced in patients with renal or hepatic impairment; caution is required.
Overdose effects

Storage conditions
Do not store above 25°C. Do not freeze. Use within one month of first use.
