Description

This preparation is a mixture of antitussive, decongestant and antihistamine agent. Phenylephrine is a decongestant that shrinks blood vessels in the nasal passage. Triprolidine is a potent competitive histamine H1-receptor antagonist which may cause drowsiness.

Pharmacology

Dextromethorphan is a safe, effective, non-narcotic antitussive agent which has a central action on the cough centre in the medulla. Mechanism of action

Dextromethorphan acts on the cough centre to suppress cough; Phenylephrine constricts nasal blood vessels to reduce congestion; Triprolidine blocks histamine receptors to relieve allergy symptoms.

Dosage

Adults & Children (≥12 years): 1 teaspoonful (5 ml) every 4 hours.
Children (6-12 years): ½ teaspoonful (2.5 ml) every 4 hours.
Children (2-6 years): Use with caution and under medical advice.
Children under 2 years: Do not use.

Administration

Take with or without food (take with food if stomach upset occurs). Measure liquid doses carefully.

Missed dose

If you miss a dose, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose. Do not double the dose.

Side effects

Common: Drowsiness, dizziness, constipation, GI discomfort.
Other: Transient hypertension, dry mouth, restlessness, palpitations, allergic reactions (rash), chest tightness, thickening of bronchial secretions, toxic psychosis, blood dyscrasia.

Precautions & warnings

May cause drowsiness; do not drive or operate machinery. Avoid alcohol. Caution in epilepsy, prostatic hypertrophy, glaucoma, liver disease, hypertension, heart disease, diabetes, hyperthyroidism, stenosing peptic ulcer, pyloro-duodenal obstruction, bladder neck obstruction. Consult doctor if no improvement within 1 week or with high fever.

Contraindications

Liver disease, asthmatic patients; patients taking MAOIs or within 2 weeks of stopping; hypersensitivity; acute asthma attacks; severe hypertension or severe coronary artery disease.

Drug interactions

MAOIs (isocarboxazid, methylene blue, moclobemide, phenelzine, procarbazine, rasagiline, selegiline, tranylcypromine): Serious (possibly fatal) interaction.
Topical antihistamines, antispasmodics, beta-blockers, Parkinson's drugs, guanethidine, inhaled anesthetics, methyldopa, reserpine, scopolamine, tricyclic antidepressants, anti-seizure drugs, sleep/anxiety medicines, muscle relaxants, narcotic pain relievers, psychiatric medicines: Caution required.

Food interactions

Take with or without food. Avoid alcohol.

Use in pregnancy

Use during pregnancy is best avoided.

Pediatric use

Do not use in children under 2 years. Use with caution in children 2-6 years under medical advice.

Overdose effects

Overdose may cause respiratory depression, paranoid psychosis, delusions, hallucinations, convulsions; phenylephrine overdose may cause hypertension and reflex bradycardia. Hospital admission is strongly advised.

Store in a cool & dry place, protected from light. Keep out of reach of children.
