Description

Sodium phosphate draws water into the lumen of the gut where it causes osmotic effect; causes abdominal distention and promotes peristalsis and evacuation of the bowel. It acts as a saline cathartic.

Pharmacology

Sodium phosphate is a saline cathartic that draws water into the intestinal lumen, creates osmotic effect, causes abdominal distention, and promotes peristalsis and bowel evacuation.

Mechanism of action

Diabasic Sodium Phosphate + Monobasic Sodium Phosphate draws water into the bowels, softens stool, and promotes bowel movement.

Dosage

Adult (Oral Tablets): Evening before colonoscopy: 4 tablets with 8 oz clear liquid every 15 min (total 20 tablets); Day of procedure: 4 tablets with 8 oz clear liquid every 15 min (total 12 tablets).
Adult (Rectal Enema): 1 bottle PR; not to exceed 1 administration/24 hr.
Children (2-4 years): Half bottle of pediatric enema (~30 mL) PR.
Children (5-11 years): 1 bottle of pediatric enema (59 mL) PR.
Children (≥12 years): Same as adults.

Administration

Remove protective shield from enema tip, gently insert tip into rectum (pointing toward navel). Squeeze bottle until nearly all liquid is gone. Maintain position for 1-5 minutes (do not retain for more than 10 minutes).

Missed dose

If you miss a dose, use it as soon as possible. However, if it is almost time for your next dose, skip the missed dose.

Side effects

Common: Nausea, vomiting, abdominal pain, abdominal distension, diarrhea, gastrointestinal pain.
Serious: Dehydration, hyperphosphatemia, hypocalcemia, hypokalemia, hypernatremia, metabolic acidosis, hypersensitivity, pruritus.

Precautions & warnings

Use with caution in patients with renal impairment, cirrhosis, cardiac failure, or with edema-causing medications. Should not be used with sodium retaining medications. Caution in patients receiving corticosteroids or corticotropin. Caution during lactation.

Contraindications

Diseases where high sodium, high phosphorus or low calcium levels may be encountered; allergy/hypersensitivity; acute phosphate nephropathy; GI obstruction; gastric bypass surgery; bowel perforation; toxic colitis; toxic megacolon; children under 2 years.

Drug interactions

Calcium channel blockers, diuretics, lithium: May affect electrolyte levels (hyperphosphatemia, hypocalcemia, hypokalemia, hypernatremic dehydration, acidosis).
Lithium: May cause fall in serum lithium levels.
Do not use with other sodium phosphate preparations concomitantly.

Food interactions

Drink large amounts of clear liquids to prevent dehydration.

Use in pregnancy

Pregnancy Category C. No adequate data available; should only be used as directed by a physician.

Pregnancy Category: C — always consult a doctor before taking during pregnancy.

Use in lactation

Sodium phosphate may pass into breast milk; it is advised to express and discard breast milk for at least 24 hours after receiving this medicine.

Pediatric use

Do not use in children under 2 years of age.

Geriatric use

Use with caution in elderly patients.

Renal / hepatic impairment

Use with caution in patients with renal impairment.

Overdose effects

Overdose may cause dehydration and electrolyte imbalance; rehydration and medical intervention required.

Store below 25°C. Protect from light. Do not refrigerate. Keep out of reach of children.
