Fluticasone Propionate Inhalation

Introduction: Fluticasone Propionate Inhalation is a steroid that relieves the symptoms of allergic rhinitis such as runny nose, sneezing and sinus discomfort. It works by blocking the production of certain chemical messengers that cause inflammation and allergies.

How it works: Fluticasone Propionate utilises a fluorocarbothioate ester linkage at the 17 carbon position. It has potent vasoconstrictive and anti-inflammatory activity, but weak HPA inhibitory effect when applied topically.

Important Information: This inhalation should be used regularly for full benefit. Normally, blow your nose gently before using it, keep one nostril closed while applying the medicine into the other and then sniff hard to make sure it reaches deep into the nose.

Fluticasone Propionate is a synthetic, trifluorinated corticosteroid that provides anti-inflammatory activity. It utilises a fluorocarbothioate ester linkage at the 17 carbon position. It has potent vasoconstrictive and anti-inflammatory activity, but weak HPA inhibitory effect when applied topically. Fluticasone has shown to exhibit anti-inflammatory effect on neutrophils, eosinophils, macrophages, mast cells, lymphocytes, and mediators (histamine, leukotrienes, cytokines, eicosanoids).

Fluticasone Propionate binds to glucocorticoid receptors and reduces the activity of inflammatory cells and mediators, resulting in decreased lung and nasal inflammation and allergy symptom control.

Adult Dose (Asthma prophylaxis):

• Powder or aerosol inhaler: 100 mcg bid in mild asthma, up to 500-1,000 mcg bid in severe cases.
• Nebuliser suspension (16+ years): 0.5-2 mg twice daily. Initial dose: 2 mg twice daily.

Child Dose (4-16 years):

• 50-100 mcg bid, increased to 200 mcg bid if necessary.
• Under 4 years: Safety and efficacy not established.
• Nebuliser suspension (4-16 years): 1 mg twice daily.

For inhalation only. Use in the dose and duration as advised by your doctor. Check the label for directions before use. Avoid deep breathing while using the nasal spray (medication may go to throat). Do not share the bottle with anyone else.

If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and follow your regular schedule.

Common Side Effects:

• Nasal irritation
• Nosebleeds
• Headache

Precautions:

• Avoid in patients with recent nasal ulcers, nasal surgery, or nasal trauma.
• Do not use for primary treatment of status asthmaticus or acute bronchospasm.
• Long-term use may require regular medical tests.

Contraindications:

• Hypersensitivity to drug, components or milk proteins (may cause anaphylaxis, angioedema, rash, urticaria)
• Primary treatment of status asthmaticus, acute bronchospasm

Drug Interactions: CYP3A4 inhibitors (e.g., ritonavir) may increase plasma concentrations of fluticasone.

Food Interactions: No interaction found/established.

Use in Pregnancy: May be unsafe during pregnancy. Animal studies have shown harmful effects. Your doctor will weigh the benefits and risks.

Use in Lactation: Probably safe during breastfeeding. Limited data suggests the drug does not represent any significant risk to the baby.

Pediatric Use: Dosage available for children 4-16 years. Safety and efficacy not established for children under 4 years.

Geriatric Use: No specific dosage adjustment is necessary for elderly patients.

Renal Impairment: No interaction found/established.

Hepatic Impairment: No interaction found/established.

Overdose: Limited information available on overdose. Employ usual supportive measures.
