Hartmann's Solution is an electrolyte and fluid replacer used for hypovolemia and dehydration.
Description
Introduction: Hartmann's Solution is a sterile electrolyte solution containing Sodium Chloride, Potassium Chloride, Calcium Chloride and Sodium Lactate. It is used to treat hypovolemia caused by surgery, hemorrhage and trauma, and dehydration.
How it works: This solution restores fluid and electrolyte balance, increases blood volume and helps treat metabolic acidosis associated with dehydration.
Important Information: The volume and rate of infusion depend on the patient's age, weight and clinical condition. For burn patients, the Parkland formula is used.
Indications
- Hypovolemia: caused by surgery, hemorrhage and trauma
- Dehydration: due to excessive sweating, severe diarrhea or vomiting, excess fluid loss by kidneys
- Plasma loss: caused by intestinal obstruction, burns or denuding skin conditions
- Metabolic acidosis: associated with dehydration (alternative to Sodium Bicarbonate)
- Alkalinized urine

Pharmacology
Hartmann's Solution is a sterile solution of Sodium Chloride, Potassium Chloride, Calcium Chloride and Sodium Lactate. Each litre contains Sodium 131 mmol, Potassium 5 mmol, Calcium 2 mmol, Bicarbonate (as lactate) 29 mmol, and Chloride 111 mmol. It restores fluid and electrolyte balance, increases blood volume and helps treat metabolic acidosis associated with dehydration.
Mechanism of action
Hartmann's Solution replenishes fluid and electrolyte deficits and restores blood volume.
Dosage
Adult Dose:
- Recommended flow rate: up to 100 drops/minute/70 kg body weight
- Burn patients (Parkland formula): 4 ml/kg × % BSA burn (over 24 hours)
- Example: 60 kg with 30% BSA burn = 7200 ml/24 hours (half in first 8 hours)
Administration
Intravenous (IV) infusion. Check infusion set and solution prior to use. Do not use if the solution is cloudy, contains particles, or after expiry date.
Missed dose
If you miss a dose, consult your doctor.
Side effects
Common Side Effects:
- Mild swelling of hands, ankles and feet (fluid retention)
- Rare: fluid in lungs causing breathing difficulty
- Nausea, vomiting
- Headache, dizziness
- Drowsiness, confusion
- Inflammation or swelling of veins around injection site
Precautions & warnings
Precautions:
- Should not be administered rapidly or for prolonged periods
- Use with caution in pregnancy, renal impairment, heart failure, pulmonary congestion
- Caution in patients receiving potassium sparing diuretics
- Discontinue if rigor arises during infusion
- Caution in diabetes mellitus (one isomer of lactate is gluconeogenic)
Contraindications
Contraindications:
- Severe hyperkalemia
- Severe hypernatremia
- Severe hyperchloremia
- Diabetes mellitus - caution required (due to lactate)
Drug interactions
Drug Interactions:
- Caution with potassium-sparing diuretics
Food interactions
Food Interactions: Not applicable (intravenous administration).
Use in pregnancy
Use in Pregnancy: Pregnancy Category C. Use only if clearly needed.

Use in lactation
Use in Lactation: Safe to use during breastfeeding.

Pediatric Use: Dose is individualized based on age, weight and clinical condition.
Geriatric use
Geriatric Use: Dose is individualized considering heart and kidney function.
Renal / hepatic impairment
Renal Impairment: Use with caution in renal impairment.
Hepatic Impairment: Use with caution in liver disease (lactate metabolism occurs in liver).
Overdose effects
Overdose: Fluid and electrolyte overload may occur. Heart failure, pulmonary edema may occur - discontinue infusion and provide supportive care.

Storage: Store at controlled room temperature.
