Description
Ketorolac Tromethamine 0.5% Eye Prep is an NSAID that inhibits prostaglandin synthesis by decreasing cyclooxygenase enzyme activity. It is used for temporary relief of ocular itching from seasonal allergic conjunctivitis, pain and inflammation following cataract surgery, and pain/burning following corneal refractive surgery.

- Allergic conjunctivitis (ocular itching from seasonal allergic conjunctivitis)
- Postoperative eye inflammation (cataract surgery)
- Ocular pain and burning/stinging following corneal refractive surgery
- Cystoid macular edema

Pharmacology
Ketorolac Tromethamine inhibits prostaglandin synthesis by decreasing cyclooxygenase enzyme activity, which helps reduce ocular pain, inflammation, and allergic reactions.
Mechanism of action
Inhibits prostaglandin synthesis by decreasing cyclooxygenase enzyme activity.
Dosage
Allergic Conjunctivitis (Adult & >2 years):
- Instill 1 drop in affected eye(s) 4 times daily (q6hr)
Post-Cataract Surgery Pain & Inflammation (Adult & >2 years):
- Instill 1 drop in affected eye(s) 4 times daily
- Start 24 hours after surgery and continue for 4 days (adult) or 2 weeks (children)
Post-Corneal Refractive Surgery Pain & Burning/Stinging (Adult & >2 years):
- Instill 1 drop in operated eye PRN
- Continue for up to 4 days postoperatively
Children (<2 years): Safety and efficacy not established.
Administration
- For ophthalmic use only
- Shake the bottle well before use
- Remove contact lenses before application and reinsert after 15 minutes
- Allow at least 5 minutes interval between different eye medications
Missed dose
Not generally applicable (topical ophthalmic use); consult doctor if needed.
Side effects

- Transient ocular burning/stinging

- Conjunctival hyperemia
- Corneal edema
- Iritis
- Corneal infiltrates
- Ocular inflammation, irritation, pain
- Increased ocular pressure
- Superficial keratitis
<1%:
- Blurred vision
- Corneal ulcer
- Corneal erosion
- Corneal thinning
- Perforation of cornea
- Dry eyes
- Epithelial breakdown
Precautions & warnings
- Discontinue use if signs of corneal epithelial breakdown occur
- Do not use while wearing contact lenses
- Monitor corneal health closely
- Use with caution in elderly, hepatic dysfunction, heart failure
- Monitor renal function in mild renal impairment
Contraindications
- Hypersensitivity to NSAIDs or any components of Ketorolac Tromethamine
- History of asthma, nasal polyp, angioedema
- History of peptic ulcer and bleeding
- Bleeding disorders
Drug interactions
- May reduce effects of antihypertensives such as ACE inhibitors or AIIA
- Increased risk of renal toxicity with ACE inhibitors, diuretics
- Increased adverse effects with aspirin or other NSAIDs
- Hallucinations may occur with fluoxetine, thiothixene, alprazolam
- Increased risk of GI bleeding with warfarin (potentially fatal)
- May increase toxicity of methotrexate (MTX) and lithium
Food interactions
No food interactions applicable (topical ophthalmic use).
Use in pregnancy
Use with caution during pregnancy; systemic absorption is very low.
Use in lactation
Can be used during lactation (systemic absorption is very low).
Pediatric use
Under 2 years: Safety and efficacy not established; over 2 years: Doses are established.
Geriatric use
Use with caution in elderly; no special dose adjustment needed.
Renal / hepatic impairment
Use with caution in hepatic dysfunction or mild renal impairment.
Overdose effects
Overdose unlikely with topical ophthalmic use; symptoms may be similar to side effects.
Storage conditions
Close the bottle immediately after use; do not use after 4 weeks of opening; store in a cool and dry place away from light; keep out of reach of children.
