Levofloxacin 0.5% Eye Prep is an antibiotic used in the treatment of bacterial eye infections. It relieves symptoms by stopping the further growth of causative microorganisms.
Description
Levofloxacin eye drops is a sterile topical ophthalmic solution. It is a synthetic, broad-spectrum, third-generation fluoroquinolone bactericidal antibiotic active against a broad spectrum of Gram-positive and Gram-negative ocular pathogens.
Indications
- Bacterial conjunctivitis
- Bacterial corneal ulcer
- Blepharitis, hordeolum, perioperative conditions
- Otitis externa and otitis media (ear drop form)

Pharmacology
Levofloxacin inhibits bacterial topoisomerase IV and DNA gyrase to prevent bacterial DNA replication, transcription, repair, and recombination.
Mechanism of action
Inhibits DNA gyrase and topoisomerase IV to prevent bacterial cell division and repair.
Dosage
0.5% Eye Drop (Adult and 1 year+):
- Days 1 & 2: 1-2 drops in affected eye(s) every 2 hours while awake, up to 8 times/day
- Days 3-7: 1-2 drops in affected eye(s) every 4 hours while awake, up to 4 times/day
Ear Drop (1.5%):
- Usual dose: 6-10 drops in the ear twice daily; perform ear bath for about 10 minutes after application
Administration
- For ophthalmic/otic use only
- Hold dropper close to the eye without touching; place drop in the lower eyelid
- Wipe off extra liquid
- Apply pressure on the corner of the eye (close to the nose) for about 1 minute after instillation
- Do not touch the dropper tip to any surface or eye (to avoid contamination)
- Use within 4 weeks of opening
Missed dose
If you miss a dose, take it as soon as possible. If it's almost time for your next dose, skip the missed dose and go back to your regular schedule. Do not double the dose.
Side effects
Common Side Effects (1-3%):
- Transient blurred vision
- Fever
- Foreign body sensation
- Headache
- Transient ocular burning
- Ocular pain or discomfort
- Photophobia
<1%: Allergic reactions, lid edema, ocular dryness, ocular itching
Precautions & warnings
- Caution when driving or using machines (may cause transient blurred vision)
- Do not wear contact lenses until infection clears up
- Avoid prolonged use to prevent overgrowth of non-susceptible organisms
Contraindications
- History of hypersensitivity to Levofloxacin, other quinolones, or any components
Drug interactions
- Additive effects with QT-prolonging drugs
- Sucralfate, antacids, zinc/calcium/iron supplements reduce absorption

Use in pregnancy
Use in Pregnancy: Levofloxacin 0.5% Eye Prep is unsafe during pregnancy (Category C); use only if potential benefit justifies potential risk to the fetus.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
Use in Lactation: Levofloxacin 0.5% Eye Prep is probably unsafe during breastfeeding; caution should be exercised.
Pediatric use
Safety not established in children under 1 year; dose established for children 1 year+.
Geriatric use
No special precautions needed in elderly.
Renal / hepatic impairment
