Description
Monobasic Sodium Phosphate + Dibasic Sodium Phosphate is a saline laxative administered via the rectal route. Fluid accumulation in the lower bowel produces distension and promotes peristalsis and bowel movement on the rectum, sigmoid and descending colon. These phenomena result in rapid evacuation.
Indications
- Relief of occasional constipation
- Bowel cleansing before and after lower bowel surgery
- Bowel cleansing during delivery and post-partum
- Before proctoscopy, sigmoidoscopy or colonoscopy
- Before radiological examinations of the lower bowel

Pharmacology
Monobasic Sodium Phosphate + Dibasic Sodium Phosphate administered rectally accumulates fluid in the lower bowel producing distension and stimulating peristalsis and bowel movement, resulting in rapid evacuation.
Mechanism of action
Stimulates bowel movement by osmotic fluid accumulation in the bowel.
Dosage
Adults, Elderly and Children >12 years: 133 ml (118 ml delivered dose) not more than once daily
Children 5-12 years: 66 ml (59 ml delivered dose) once daily
Children 2-5 years: 66 ml (59 ml delivered dose) once daily
Additional fluids: Encourage patients to drink large amounts of clear liquids to prevent dehydration.
Administration
- For rectal use only
- Shake well before use
- Do not exceed recommended dose
- Do not use more than 133 ml in 24 hours
Missed dose
If you miss a dose, use it as soon as you remember; skip if it's almost time for next dose; do not double the dose.
Side effects
Common Side Effects:
- Rectal irritation/burning
- Abdominal pain or cramps
- Nausea
- Electrolyte imbalance (hyperphosphatemia, hypocalcemia, hypokalemia)
- Dehydration
Precautions & warnings
- Caution in patients taking calcium channel blockers, diuretics, lithium or medications affecting electrolyte levels
- Caution in renal impairment
- Caution in pre-existing electrolyte disturbances
- Caution in patients taking QT-prolonging medications
- Caution in ascites
- Caution in patients with colostomy
Contraindications
- Congestive heart failure
- Renal impairment
- Gastrointestinal obstruction
- Megacolon
- Paralytic ileus
- Perforation
- Active inflammatory bowel disease
- Imperforate anus
- Dehydration
- Children under 2 years
- Hypersensitivity
Drug interactions
- Caution with calcium channel blockers, diuretics, lithium (may cause electrolyte imbalance)
- Do not use concomitantly with other sodium phosphate preparations
- May decrease serum lithium levels with lithium therapy

Use in pregnancy
Use in Pregnancy: No adequate data available to evaluate potential for fetal malformation or feto-toxic effects. Use during delivery or postpartum as directed by physician.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
Use in Lactation: Sodium phosphate may pass into breast milk. It is advised to express and discard breast milk for at least 24 hours after receiving this medicine.
Pediatric use
Contraindicated in children under 2 years; doses established for 2-5 years and 5-12 years.
Geriatric use
Elderly may require dose adjustment (based on renal function).
Renal / hepatic impairment
Renal Impairment: Caution in kidney disease; contraindicated in severe renal insufficiency.
Hepatic Impairment: Caution in liver disease; dose adjustment may be needed.
Overdose effects
Overdose may cause electrolyte imbalance, hyperphosphatemia, hypocalcemia, hypokalemia, dehydration; rehydration and electrolyte replacement therapy required.
Storage conditions
Store below 25°C; protect from light; do not refrigerate; keep out of reach of children.
