At a glance

Refined Soyabean Oil + Purified Egg Lecithin + Glycerol is an isotonic fat emulsion used as a source of energy as part of parenteral nutrition. It is used for preoperative and postoperative nutritional disturbances, burns, prolonged unconsciousness, impaired renal function, and essential fatty acid deficiency.

Description

This fat emulsion contains soy oil, egg lecithin, and glycerol and is used as a concentrated source of energy together with carbohydrates and amino acids in parenteral nutrition. It is isotonic and provides a source of basal phosphate requirements and vitamin E.

Indications
  • Parenteral nutrition (as part of TPN)
  • Preoperative and postoperative nutritional disturbances
  • Burns (to reduce excessive nitrogen losses)
  • Prolonged unconsciousness (cranial trauma, poisoning)
  • Impaired renal function (to reduce protein breakdown)
  • Essential fatty acid deficiency
Therapeutic class

Parenteral Nutrition

Pharmacological class

IV Fat Emulsion

Pharmacology

This fat emulsion provides a concentrated source of energy in parenteral nutrition. It supplies essential fatty acids (linoleic acid), meets phosphate requirements, and provides vitamin E.

Mechanism of action

Provides energy and essential fatty acids through parenteral nutrition.

Dosage

Adult Dose (IV):

  • As part of TPN: Fat emulsion should comprise no more than 60% of total caloric intake
  • Initial infusion rate: 1 ml/min for first 15-30 min; increase to 2 ml/min if no adverse reactions
  • Infuse only 500 ml the first day; increase dose the following day
  • Do not exceed daily dosage of 2.5 g/kg
  • Fatty acid deficiency correction: 8-10% of caloric intake by IV fat emulsion

Pediatric Dose (IV):

  • Initial infusion rate: 0.1 ml/min for first 10-15 min; increase to 1 g/kg in 4 hours if no adverse reactions
  • Do not exceed daily dosage of 3 g/kg
  • Infants: Start at 0.5 g/kg/24 hours; max 3 g/kg/24 hours
Administration
  • Administered by healthcare professional as IV infusion only (peripheral vein or central venous catheter)
  • Use together with carbohydrates and amino acids as part of parenteral nutrition
Missed dose

If you miss a dose, consult your doctor.

Side effects

Side Effects:

  • Rise in body temperature (incidence <3%)
  • Shivering, chills, nausea/vomiting (incidence <1%)
  • Hypersensitivity reactions (anaphylactic reaction, skin rash, urticaria)
  • Respiratory symptoms (tachypnoea)
  • Circulatory effects (hypertension, hypotension)
  • Thrombosis, hemolysis, abdominal pain, tiredness, headache, flushing, dyspnea, dizziness
Precautions & warnings
  • Soya oil and egg lecithin may cause allergic reactions; cross-allergy between soya-bean and peanut observed
  • Strong correlation between C-reactive protein and fat emulsion agglutination in seriously ill patients; administer with caution
  • Caution in neonates and premature infants with hyperbilirubinaemia and suspected pulmonary hypertension
  • In low birthweight infants, risk of lipid infusions may outweigh benefits due to diminished infection defence
  • Monitor platelet count, liver function tests, and serum triglyceride in neonates on long-term parenteral nutrition
Contraindications
  • Severely disordered fat metabolism (severe liver damage, acute shock)
  • Hypersensitivity to egg-, soya-, or peanut protein or to active substances/excipients
Drug interactions

No known interactions.

Food interactions

Not food-related (IV administration); it is not known whether it is safe to consume alcohol.

Use in pregnancy

Use in Pregnancy: Absolute safety of the fetus has not been established; administer with caution during pregnancy.

Use in Lactation: Absolute safety of the nursing infant has not been established; administer with caution during lactation.

Pediatric use

Pediatric dose based on weight; caution in premature infants.

Geriatric use

Caution required in elderly patients; dose adjustment may be needed.

Renal / hepatic impairment

Renal Impairment: Indicated in impaired renal function; caution; consult your doctor.

Hepatic Impairment: Contraindicated in severe liver damage; fat metabolism may be disturbed in liver insufficiency; consult your doctor.

Overdose effects

Overdose: Fat overload syndrome (hyperlipidemia, liver function abnormalities, platelet decrease); treatment: stop infusion, symptomatic supportive care.
