Tirofiban Hydrochloride is a glycoprotein IIb/IIIa receptor antagonist used to reduce the rate of thrombotic cardiovascular events in patients with non-ST elevation acute coronary syndrome (NSTE-ACS).

Description

Tirofiban is a reversible antagonist of fibrinogen binding to the GP IIb/IIIa receptor, the major platelet surface receptor involved in platelet aggregation. When administered intravenously, it inhibits ex vivo platelet aggregation in a dose- and concentration-dependent manner. Platelet aggregation inhibition is reversible following cessation of the infusion.

Pharmacology

Tirofiban binds to the GP IIb/IIIa receptor, blocking fibrinogen binding and preventing platelet aggregation. IV administration achieves >90% inhibition of platelet aggregation within 10 minutes, which is reversible upon discontinuation.

Mechanism of action

Prevents platelet aggregation by blocking GP IIb/IIIa receptors.

Dosage

Adult Dose (IV):

  • 25 mcg/kg over 5 minutes, then 0.15 mcg/kg/min (or 0.075 mcg/kg/min if serum creatinine ≤60 mL/min), for up to 18 hours
Administration
  • IV administration only
  • Administered by a healthcare professional in a hospital
  • Do not use if solution is not clear or seal is broken
  • Do not administer through the same IV line as diazepam

Most Common Side Effect:

  • Bleeding
Precautions & warnings
  • Bleeding risk: Minimize traumatic procedures (arterial and venous punctures, IM injections)
  • Thrombocytopenia: Monitor platelet counts 6 hours after initiation and daily; discontinue tirofiban and heparin if <90,000/mm³
  • Increased bleeding risk with fibrinolytics, anticoagulants, antiplatelets
Contraindications
  • Severe hypersensitivity to tirofiban (anaphylaxis)
  • History of thrombocytopenia following prior tirofiban exposure
  • Active internal bleeding or bleeding diathesis
  • Major surgical procedure or severe physical trauma within the previous month
Drug interactions
  • Antiplatelets, thrombolytics, heparin, aspirin: Increased bleeding risk
Food interactions

Alcohol consumption is not relevant (injection).

Use in pregnancy

Use in Pregnancy: No fetal harm in animal studies; adequate human data not available; use only if clearly needed; consult your doctor.

Pregnancy Category: B — always consult a doctor before taking during pregnancy.

Use in lactation

Use in Lactation: Unknown if excreted in human milk; significant levels in rat milk; decide whether to discontinue nursing or the drug; consult your doctor.
