Description
Imipenem + Cilastatin is a combination of antibiotic medicines. It is prescribed to treat various types of severe bacterial infections. It fights against the infection-causing microorganisms to prevent further infections. Imipenem + Cilastatin should only be administered under the supervision of a healthcare professional. Do not miss any scheduled appointments for taking this medicine. The course of the medicine should be completed even if you feel better. The common side effects of this medicine are nausea, vomiting, diarrhea, rashes, etc. You may also experience injection site redness, swelling, and pain that goes away with time. If any of the side effects get aggravated, you must consult your doctor immediately. If you experience any allergic reaction (rashes, itching, swelling, shortness of breath, etc.), you must seek immediate medical help. Before taking this medicine, you should tell your doctor if you are taking any medicines for any health condition. Pregnant or breastfeeding women should also consult with their doctor before taking it. Avoid drinking alcohol as it can cause excessive drowsiness with this medicine. It usually does not impair your ability to drive, but you should not drive if it makes you feel sleepy or dizzy. Imipenem + Cilastatin is a combination of two medicines: Imipenem and Cilastatin. Imipenem is an antibiotic. It works by preventing the formation of the bacterial protective covering which is essential for the survival of bacteria in the human body. Cilastatin is dehydropeptidase I inhibitor which blocks the activity of an enzyme (dehydropeptidase I) that causes breakdown of Imipenem. Hence, it helps Imipenem stay active in your body for a longer period of time.

Mechanism of action
Imipenem is a potent inhibitor of bacterial cell wall synthesis and is bactericidal against a broad spectrum of pathogens. It is resistant to degradation by bacterial beta-lactamases. Cilastatin is an inhibitor of dehydropeptidase I, an enzyme found in the brush border of the renal tubules. It is given as the sodium salt with imipenem to prevent its renal metabolism and protect against nephrotoxic effects.
Dosage
Adult Dose: Lower Respiratory Tract, Skin/Skin Structure, & Gynecologic Infections Mild to moderate: 500-750 mg IV q12hr Intra-abdominal Infections Mild to moderate: 250-500 mg IV q6hr Severe: 500 mg IV q6hr or 1 g q8hr for 4-7 days, provided that infection is brought under control Pseudomonas Infections 500 mg IV q6hr; higher dosages may be administered, depending on organism sensitivity Urinary Tract Infections Uncomplicated: 250 mg IV q6hr Complicated: 500 mg IV q6hr Susceptible infections Moderate infections IV: Fully susceptible organisms: 500 mg IV q6-8hr Moderately susceptible organisms: 500 mg IV q6hr or 1 g IV q8hr Severe infections Fully susceptible organisms: 500 mg IV q6hr Moderately susceptible organisms: 1 g IV q6-8hr; not to exceed 50 mg/kg/day or 4 g/day, whichever is lower Child Dose: Child: IV, IM 60–100 mg/kg/day q6h IM form not approved for <12 y Renal Dose: Renal impairment: CrCl (ml/min) 31-70 500 mg every 6-8 hr 21-30 500 mg every 8-12 hr 6-20 250 mg or 3.5 mg/kg (whichever is lower) every 12 hr <5 Only give if haemodialysis started within 48 hr

Precautions & warnings
[UNSAFE] Imipenem + Cilastatin may cause side effects which could affect your ability to drive. There are some side effects associated with this medicine (such as seeing, hearing, or feeling something that is not there) that may affect some patients' ability to drive. Imipenem + Cilastatin is used to treat severe bacterial infections, including those acquired in hospital and those involving many different types of bacteria. It is given by a drip into a vein (intravenous infusion) over 20 to 60 minutes. Tell you doctor if you are allergic to penicillin as this medicine should be avoided or used with caution in penicillin allergy. Your doctor may monitor your liver and kidney function while you are having treatment with this medication. Diarrhea may occur as a side effect but should stop when your course is complete. Inform your doctor if it doesn't stop or if you find blood in your stools. Inform your doctor if you are pregnant, planning to conceive, or breastfeeding.
Contraindications
Hypersensitivity.
Drug interactions
Concurrent admin with probenecid may increase the half-life of cilastatin. Increased risk of generalised seizures when used concurrently with ganciclovir.
Food interactions

Use in pregnancy
