Description
Diltiazem Hydrochloride is used to treat angina (heart-related chest pain), high blood pressure and some types of irregular heartbeats (arrhythmia). It is known as a calcium channel blocker. It works by relaxing blood vessels to lower blood pressure. This helps to reduce the workload of the heart. Diltiazem Hydrochloride may be taken with or without food, but it is better to take it regularly at a fixed time each day as advised by your doctor. Keep using this medicine even if you feel well. If you stop taking it suddenly, your condition may worsen. This medicine is only part of a treatment program that should include a healthy diet, regular exercise, and weight reduction as advised by your doctor. Common side effects seen with this medicine include swelling of the ankles or feet (edema), dizziness, headache, nausea, constipation, flushing, rash and fatigue. These are usually mild and disappear after a short time. Consult your doctor if they bother you or do not go away. It may also make you feel sleepy or dizzy, so be careful if you drive or do anything that requires you to be alert. Drinking alcohol should be avoided while taking this medicine as it may worsen the side effects. Before taking this medicine, let your doctor know if you have any liver or kidney problems. Also, inform your doctor if you have very low blood pressure (hypotension), heart failure, or if you recently had a heart attack and have fluid in your lungs. Your doctor may want to monitor your blood pressure while using it and you may need frequent blood tests. Pregnant or breastfeeding women should consult their doctor before taking it. Diltiazem Hydrochloride is a calcium channel blocker. It blocks the action of calcium on the heart and blood vessels, thereby widening the blood vessels and allowing more blood and oxygen to reach the heart. This reduces blood pressure and decreases the chances of angina (chest pain).

Mechanism of action
Diltiazem relaxes coronary vascular smooth muscles by inhibiting influx of calcium ions during depolarisation of the vascular smooth muscles and myocardium. It increases myocardial O2 delivery in patients with vasospastic angina and inhibits cardiac conduction, particularly at the SA and AV nodes.
Dosage
Adult Dose: Angina Conventional tablet: 30 mg PO q6hr; increased every 1 or 2 days until angina controlled (usually 180-360 mg/day PO divided q6-8hr); not to exceed 360 mg/day SR: 120 mg/day PO; titrate after 7-14 days; maintenance range usually 120-320 mg/day; not to exceed 540 mg/day Hypertension Conventional tablet: Initial: 60-120 mg twice daily. Max: 360 mg/day. SR: Twice-daily dosing: 60-120 mg PO q12hr; may be adjusted after 14 days; maintenance range usually 240-360 mg/day Hepatic Impairment Use caution Renal Dose: Renal Impairment Use caution
Administration
Take this medicine in the dose and duration as advised by your doctor. Swallow it as a whole. Do not chew, crush or break it. Diltiazem Hydrochloride may be taken with or without food, but it is better to take it at a fixed time.

Precautions & warnings
[UNSAFE] Diltiazem Hydrochloride may decrease alertness, affect your vision or make you feel sleepy and dizzy. Do not drive if these symptoms occur. Dilzem 30 Tablet CR slows down the heartbeat and allows the blood vessels in the body to relax. This helps prevent angina attacks (chest pain caused by heart disease), certain abnormal heart rhythms, stroke and heart attack. It may cause dizziness or sleepiness. Do not drive or do anything requiring concentration until you know how it affects you. It may cause you to feel lightheaded or weak especially when you stand up (orthostatic hypotension). Rise slowly if you have been sitting or lying down. It is best to avoid drinking alcohol while taking Dilzem 30 Tablet CR as it may make the side effects worse. Do not stop taking it suddenly without talking to your doctor first.
Contraindications
Sick-sinus syndrome; 2nd or 3rd ° AV block; porphyria. Severe congestive cardiac failure; marked bradycardia. Pregnancy and lactation.
Drug interactions
Increased AV blocking effect w/ clonidine. May increase serum levels of statins (e.g. atorvastatin, lovastatin), carbamazepine, phenytoin. May increase bradycardic affect of amiodarone, digoxin, mefloquine. May enhance antihypertensive effect w/ other antihypertensives (e.g. aldesleukin), antipsychotic. Increased serum levels w/ cimetidine. Decreased serum levels w/ rifampicin, phenobarbital. Potentially Fatal: IV: May enhance hypotensive effect of IV ?-blockers.
Food interactions

Use in pregnancy

Use in lactation
[SAFE IF PRESCRIBED] Diltiazem Hydrochloride is safe to use during breastfeeding. Human studies suggest that the drug does not pass into the breastmilk in a significant amount and is not harmful to the baby.
Renal / hepatic impairment
[SAFE IF PRESCRIBED] Diltiazem Hydrochloride is safe to use in patients with kidney disease. No dose adjustment of Diltiazem Hydrochloride is recommended. However, talk to your doctor if you have any underlying kidney disease. Regular monitoring of kidney function test is advised while you are taking this medicine. [CAUTION] Diltiazem Hydrochloride should be used with caution in patients with liver disease. Dose adjustment of Diltiazem Hydrochloride may be needed. Please consult your doctor.

Related to chest pain angina commonly I m asthamatic ( No attack since 30 yrs ) I take Nebistar2.5 and Dilzem CD 120, twice a day but still my BP remains 155/96. I dont smoke, drink or take nonveg and greasy food. use only cow ghee 1TSF and mustard oil. All test are normal except little rise in SGPT/SGOT.
