Description
Adefovir Dipivoxil is a diester prodrug of adefovir, an acyclic nucleotide analog with potent activity against hepatitis B virus (HBV) by inhibiting viral DNA synthesis.
Indications
Treatment of chronic hepatitis B in adults with evidence of active viral replication and elevated serum aminotransferases (ALT or AST) or histologically active disease.

Pharmacology
Adefovir dipivoxil is converted intracellularly to adefovir diphosphate. It competitively inhibits HBV DNA polymerase (reverse transcriptase) and causes termination of the viral DNA chain. Oral bioavailability is ~59% and renal excretion is the primary elimination route.
Mechanism of action
Inhibits HBV DNA polymerase by competing with natural deoxyadenosine triphosphate and inducing DNA chain termination after incorporation into viral DNA.
Dosage
Adults (normal renal function): 10 mg once daily. Renal Impairment: CrCl 20-49 mL/min: 10 mg every 48 hours; CrCl 10-19 mL/min: 10 mg every 72 hours; Hemodialysis: 10 mg every 7 days post-dialysis.
Administration
For oral use. Administer once daily with or without food.
Missed dose
Take as soon as remembered unless it is almost time for the next dose. Do not double the dose.
Side effects
Common: Asthenia, headache, abdominal pain, nausea. Serious: Nephrotoxicity, severe acute exacerbation of hepatitis B upon discontinuation, lactic acidosis, and hepatomegaly with steatosis.
Precautions & warnings
Monitor renal function closely in at-risk patients and adjust dose. Perform periodic liver function monitoring post-discontinuation.
Contraindications
Known hypersensitivity to adefovir dipivoxil or any component of the formulation.
Drug interactions
Coadministration with ibuprofen (800 mg tid) increases adefovir AUC by 23% and Cmax by 33%. No significant interactions with lamivudine, TMP-SMX, or acetaminophen.
Food interactions
Can be taken without regard to food.
Use in pregnancy
Pregnancy Category C. Use during pregnancy only if potential benefit justifies potential risk to fetus.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
Excretion in human milk is unknown. Mothers should be instructed not to breastfeed while taking adefovir dipivoxil.
Pediatric use
Safety and effectiveness in pediatric patients have not been established.
Geriatric use
Use with caution in elderly patients due to higher prevalence of decreased renal or cardiac function.
Renal / hepatic impairment
Dose interval adjustment required based on baseline creatinine clearance. Monitor liver function upon discontinuation.
Overdose effects
High doses (250-500 mg daily) cause gastrointestinal adverse effects. Provide supportive and symptomatic treatment.

Store at a cool, dry place protected from light and moisture. Keep out of reach of children.
