Amphotericin B is a systemic polyene antifungal medication used to treat severe fungal infections and visceral leishmaniasis.
Indications
Empirical therapy in febrile neutropenic patients, cryptococcal meningitis, refractory fungal infections, and visceral leishmaniasis.
Pharmacology
Binds irreversibly to ergosterol in fungal cell membranes, disrupting membrane integrity and causing cell death.
Dosage
Test dose: 1 mg IV over 20-30 min. Load: 0.25-0.5 mg/kg IV. Maintenance: 0.25-1 mg/kg IV daily or up to 1.5 mg/kg every other day.
Side effects
Fever, chills, convulsions, nausea, vomiting, diarrhea, hypotension, hypertension, arrhythmias, renal toxicity.
Use in pregnancy
Pregnancy Category B. Use if benefits outweigh risks.
Pregnancy Category: B — always consult a doctor before taking during pregnancy.
Precautions & warnings
Must be administered by medically trained personnel under close clinical observation.
Overdose effects
Supportive care with cardiovascular and respiratory monitoring.
Storage conditions
Store unopened vials up to 25°C. Reconstituted product stable for 24 hours at 2-8°C.
