Albumin Human 25% is a hyperoncotic plasma expander used to treat reduced blood volume (hypovolemia) and low blood albumin levels (hypoalbuminemia). It increases intravascular oncotic pressure and draws fluid from interstitial spaces into the vascular compartment . The 25% solution provides approximately 5 times the oncotic effect per unit volume compared to 5% albumin, making it suitable for patients who require limited fluid and sodium intake . In Bangladesh, it is available as Albutein 25%, AlbuRx 25%, etc., in 50 ml and 100 ml bottles
Description
Albumin Human 25% is used to treat blood volume loss. It works by replacing the body fluids lost due to excessive trauma, bleeding, surgery, or kidney dialysis. Thus, it is helpful in treating low blood albumin levels. Albumin Human 25% must be administered under the supervision of a healthcare professional. You should not take it if you have any known allergy from this injection. Before taking it, inform the doctor you are suffering from any severe heart disease. Your doctor may check your blood pressure and ask you to get regular blood tests done while you are using this injection. The most common side effects include flushing (sense of warmth in the face, ears, neck, and trunk), nausea and fever. Inform your doctor if you are pregnant or breastfeeding or suffering from kidney diseases as a dose adjustment may be required. This injection is safe to use in patients suffering from liver diseases. Albumin Human 25% contains human albumin, a natural protein in blood. It works by replacing the blood or body fluids lost due to excessive bleeding, surgery, or kidney dialysis.
Indications
• Hypovolemia: To restore and maintain plasma volume when blood volume is reduced due to surgery, trauma, shock, or burns, particularly when crystalloids are inadequate. • Hypoalbuminemia: To correct low blood albumin levels caused by conditions such as liver cirrhosis, nephrotic syndrome, acute or chronic liver failure, malnutrition, and other conditions. • Paracentesis in Cirrhotic Ascites: To prevent paracentesis-induced circulatory dysfunction following the removal of large volumes of ascitic fluid. • Ovarian Hyperstimulation Syndrome (OHSS): As a plasma volume expander in severe OHSS to help prevent fluid shifts and ascites. • Adult Respiratory Distress Syndrome (ARDS): To correct fluid volume overload in combination with diuretics. • Burns: After 24 hours following a burn injury, once capillary permeability has stabilized, to assist in the reabsorption of edema. • Cardiopulmonary Bypass: As a priming fluid during extracorporeal bypass procedures to maintain colloid osmotic pressure. • Hemodialysis: In patients undergoing long-term dialysis or those with fluid overload. • Therapeutic Plasma Exchange: As a plasma replacement fluid during plasmapheresis procedures.

Pharmacology
Human albumin is not a glycoprotein and has the lowest molecular weight (66,241 Daltons) of all plasma proteins, resulting in lower viscosity than other plasma proteins . Approximately 40% of albumin is in circulation; the remainder is in extravascular spaces of tissues (principally muscle, skin, and intestine) . The major function of albumin is osmotic regulation . The 25% solution is hyperoncotic, providing higher oncotic pressure per unit volume compared to 5% albumin . Following intravenous infusion, it increases plasma volume by approximately 3.5 times within 15 minutes .
Mechanism of action
Human albumin increases intravascular oncotic pressure and causes movement of fluids from interstitial into intravascular space. Human albumin solutions are available in various concentrations. Solutions containing 5% human albumin are usually used in hypovolemic patients, whereas more concentrated 25% solutions are recommended in patients in whom fluid and sodium intake must be minimised e.g. patients with hypoproteinaemia or cerebral oedema or in paediatric patients.
Dosage
Adult Dose: Adult: Initially, 25 g of albumin, adjusted according to patient's response. Usual rates of infusion: 1-2 ml/minute. Child Dose: Child: Up to 1 g/kg, adjusted according to patient's response. Usual rates of infusion: 1-2 ml/minute. Not to exceed 250 g/48 hr or 6 g/kg/day

Side effects

Precautions & warnings
[SAFE] Albumin Human 25% does not usually affect your ability to drive. It is given as an injection into veins under the supervision of a medical professional. Inform your doctor immediately if you experience a skin rash, itching, breathing difficulties, swelling of face and changes in heartbeat while using this medicine. Inform your doctor if you have history of severe anemia (lack of red blood cells), or severe heart failure.
Contraindications
Cardiac failure, severe anaemia, history of hypersensitivity, parenteral nutrition.
Drug interactions
Albumin solution should not be mixed by protein hydrolysates or alcoholic solutions. Risk of atypical reactions to ACE inhibitors in patients undergoing therapeutic plasma exchange with albumin human replacement.
Food interactions

Use in pregnancy

Pediatric use
• In children, the dose depends on the clinical condition and body weight, usually 0.5–1 g/kg. • The infusion should be administered slowly with careful monitoring for signs of volume overload.
Geriatric use
• Use the adult dosage, but close monitoring is required due to the increased risk of cardiac and renal complications.
Renal / hepatic impairment
[CAUTION] Albumin Human 25% should be used with caution in patients with kidney disease. Dose adjustment of Albumin Human 25% may be needed. Please consult your doctor. [SAFE IF PRESCRIBED] Albumin Human 25% is safe to use in patients with liver disease. No dose adjustment of Albumin Human 25% is recommended.
Overdose effects
• Overdose may cause hypervolemia (circulatory overload), which may present with headache, shortness of breath, jugular vein distension, increased blood pressure, and pulmonary edema. • At the first signs of circulatory overload, stop the infusion and monitor hemodynamic parameters.
Storage conditions
• Store below 30°C, protected from light. • Keep in the original carton. • Do not freeze. • The solution must be clear at the time of use. • Keep out of reach of children.
