Albumin Human 25% is a hyperoncotic plasma expander used to treat reduced blood volume (hypovolemia) and low blood albumin levels (hypoalbuminemia). It increases intravascular oncotic pressure and draws fluid from interstitial spaces into the vascular compartment . The 25% solution provides approximately 5 times the oncotic effect per unit volume compared to 5% albumin, making it suitable for patients who require limited fluid and sodium intake . In Bangladesh, it is available as Albutein 25%, AlbuRx 25%, etc., in 50 ml and 100 ml bottles
Indications
• Hypovolemia: To restore and maintain plasma volume when blood volume is reduced due to surgery, trauma, shock, or burns, particularly when crystalloids are inadequate. • Hypoalbuminemia: To correct low blood albumin levels caused by conditions such as liver cirrhosis, nephrotic syndrome, acute or chronic liver failure, malnutrition, and other conditions. • Paracentesis in Cirrhotic Ascites: To prevent paracentesis-induced circulatory dysfunction following the removal of large volumes of ascitic fluid. • Ovarian Hyperstimulation Syndrome (OHSS): As a plasma volume expander in severe OHSS to help prevent fluid shifts and ascites. • Adult Respiratory Distress Syndrome (ARDS): To correct fluid volume overload in combination with diuretics. • Burns: After 24 hours following a burn injury, once capillary permeability has stabilized, to assist in the reabsorption of edema. • Cardiopulmonary Bypass: As a priming fluid during extracorporeal bypass procedures to maintain colloid osmotic pressure. • Hemodialysis: In patients undergoing long-term dialysis or those with fluid overload. • Therapeutic Plasma Exchange: As a plasma replacement fluid during plasmapheresis procedures.
Dosage
Adult Dose: Adult: Initially, 25 g of albumin, adjusted according to patient's response. Usual rates of infusion: 1-2 ml/minute. Child Dose: Child: Up to 1 g/kg, adjusted according to patient's response. Usual rates of infusion: 1-2 ml/minute. Not to exceed 250 g/48 hr or 6 g/kg/day
Side effects
Common: • Vomiting • Flushing (sense of warmth in the face, ears, neck and trunk) • Fever • Nausea • Chills • Hives • Slow heart rate
Precautions & warnings
[SAFE] Albumin Human 25% does not usually affect your ability to drive. It is given as an injection into veins under the supervision of a medical professional. Inform your doctor immediately if you experience a skin rash, itching, breathing difficulties, swelling of face and changes in heartbeat while using this medicine. Inform your doctor if you have history of severe anemia (lack of red blood cells), or severe heart failure.
