Description
Amitriptyline + Chlordiazepoxide is a combination used for moderate to severe depression with moderate to severe anxiety. Amitriptyline inhibits norepinephrine and serotonin reuptake in the brain to regulate mood. Chlordiazepoxide enhances GABA-mediated chloride influx to reduce abnormal nerve cell activity and relieve anxiety. This combination is effective for insomnia, guilt feelings, agitation, psychic and somatic anxiety, and anorexia.

Moderate to severe depression associated with moderate to severe anxiety; insomnia, feelings of guilt or worthlessness, agitation, psychic and somatic anxiety, suicidal ideation, anorexia.

Pharmacology
Amitriptyline is a tricyclic antidepressant that inhibits norepinephrine and serotonin reuptake in the brain, producing antidepressant activity. Chlordiazepoxide is an anxiolytic that enhances GABA-mediated chloride influx through GABA receptor channels, causing membrane hyperpolarization and producing sedative and anxiolytic effects.
Mechanism of action
Amitriptyline increases norepinephrine and serotonin levels in the brain to improve mood; Chlordiazepoxide enhances GABA activity to reduce abnormal nerve cell activity and relieve anxiety.
Dosage
Initial dose: 3-4 tablets daily in divided doses. The larger portion of the total daily dose may be taken at bedtime. In some patients, a single dose at bedtime may be sufficient. Maintenance: Reduce to the smallest effective dose.
Administration
May be taken with or without food. Take at the same time each day. Swallow tablet whole; do not chew.
Missed dose
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular schedule. Do not double the dose.
Side effects
Common side effects: Drowsiness, dry mouth, constipation, blurred vision, dizziness, bloating, fatigue, weakness, restlessness, lethargy, weight gain, confusion, difficulty in urination, orthostatic hypotension, increased heart rate, uncoordinated movements, memory impairment. Rare: Vivid dreams, impotence, tremor, nasal congestion.
Precautions & warnings
Do not stop suddenly without consulting your doctor (withdrawal symptoms may occur). Caution while driving (drowsiness, dizziness, blurred vision). Avoid alcohol completely. Caution in liver disease; dose adjustment may be needed. Caution in patients with history of seizures. Inform doctor if you experience suicidal thoughts or unusual mood changes. Do not use during pregnancy and breastfeeding.
Contraindications
Hypersensitivity to benzodiazepines or tricyclic antidepressants; concomitant use with MAO inhibitors; recent heart attack (recovery phase).
Drug interactions
MAO inhibitors: Concomitant use is contraindicated. Guanethidine: Blocks antihypertensive effect. CNS depressants (alcohol, barbiturates): Increased sedation. Anticholinergics: Increased anticholinergic effects. Cimetidine: May increase amitriptyline levels.
Food interactions
Alcohol consumption is strictly prohibited (increases CNS depression).
Use in pregnancy
Pregnancy Category: D. Amitriptyline + Chlordiazepoxide is unsafe during pregnancy; definite evidence of risk to the fetus. Use only if clearly needed with medical advice.
Pregnancy Category: D — always consult a doctor before taking during pregnancy.
Use in lactation
Amitriptyline + Chlordiazepoxide is excreted in breast milk; use during breastfeeding is not recommended.
Pediatric use
Safety and efficacy have not been established in children; balance potential risks with clinical need.
Geriatric use
Elderly and debilitated patients: Use the lowest effective dose to avoid ataxia, over sedation, confusion or anticholinergic effects.
Renal / hepatic impairment
Renal impairment: Probably safe; dose adjustment may not be needed. Hepatic impairment: Caution required; dose adjustment may be needed.
Overdose effects
Overdose symptoms: cardiac arrhythmias, severe hypotension, convulsions, coma, confusion, diminished reflexes. Treatment: symptomatic and supportive measures, ECG monitoring.

Store in a cool and dry place, protected from light. Keep out of reach of children.
