Description
Benzoic Acid 6% + Salicylic Acid 3% (Whitfield's ointment) is a combination topical preparation used for the treatment of fungal skin infections. This 2:1 ratio (6%:3%) combines the fungistatic action of benzoic acid with the keratolytic action of salicylic acid.
Benzoic acid exhibits weak antifungal and antibacterial properties and helps acidify urine. Salicylic acid has a potent keratolytic action that softens and destroys the stratum corneum by increasing endogenous hydration, causing the horny layer of the skin to swell, soften, and then desquamate. At high concentrations, salicylic acid has a caustic effect.
Since benzoic acid is only fungistatic, eradication of the infection occurs only after the infected stratum corneum is shed, requiring continuous medication for several weeks to months. Salicylic acid accelerates desquamation.

Pharmacology
Benzoic acid exhibits weak antifungal and antibacterial properties and helps acidify urine. Salicylic acid has a potent keratolytic action and a slight antiseptic action when applied topically. It softens and destroys the stratum corneum by increasing endogenous hydration, which causes the horny layer of the skin to swell, soften, and then desquamate.
Benzoic acid is only fungistatic, so eradication of the infection occurs only after the infected stratum corneum is shed, requiring continuous medication for several weeks to months. Salicylic acid accelerates the desquamation.
Mechanism of action
Benzoic acid enters the fungal cell wall and inhibits their growth (fungistatic). Salicylic acid accelerates the keratinization process by increasing endogenous hydration of the stratum corneum and removes the infected cell layers. Together, they effectively eliminate fungal infections.
Dosage
Adults and Children: Apply a thin layer to the affected area 2-3 times daily.
Neonates: Should not be used.
Administration
For external use only. Clean and dry the affected area and apply a small amount of ointment. Avoid contact with mouth, eyes, and other mucous membranes. Wash hands after application.
Missed dose
If you miss a dose, apply it as soon as you remember. If it is almost time for your next dose, skip the missed dose. Do not double the dose.

Precautions & warnings
- Do not use in high concentrations, on large areas of the body, or for prolonged periods
- Avoid broken skin, mouth, eyes, mucous membranes, and anogenital region
- Use with caution on the extremities of patients with impaired peripheral circulation and diabetes
- If undue skin irritation develops or increases, discontinue use and consult a physician
Contraindications
- Hypersensitivity to benzoic acid, salicylic acid, or any component
Drug interactions
- Acetazolamide, anticoagulants, dipyridamole, heparin, hypoglycemics, and methotrexate: Topical salicylic acid may be absorbed and increase their toxicity
- Bumetanide, captopril, and probenecid: Salicylic acid may decrease their efficacy
- Light kaolin: May adsorb benzoic acid and reduce its activity
Food interactions
No food-related restrictions (topical application).
Use in pregnancy
[CONSULT YOUR DOCTOR] Salicylic acid is teratogenic in animal models. Although not demonstrated in humans, use during pregnancy is not advised.
Use in lactation
[CONSULT YOUR DOCTOR] Salicylic acid is excreted in breast milk. Should not be used by breastfeeding women.
Pediatric use
Should not be used in neonates. May be used in children in appropriate doses.
Geriatric use
No dose adjustment needed for elderly patients, but caution is advised (higher risk of peripheral circulation and diabetes).
Renal / hepatic impairment
Systemic absorption with topical application is minimal, but prolonged use on large areas may cause salicylism. Caution in renal or hepatic impairment.
Overdose effects
Overuse may cause salicylism (nausea, vomiting, tinnitus, dizziness). Discontinue use and consult a doctor if symptoms occur.
Storage conditions
Store below 25°C, away from light and moisture. Keep out of reach of children.
