Description
Bromfenac Sodium is a nonsteroidal anti-inflammatory drug (NSAID) with anti-inflammatory activity. Its mechanism of action is thought to be due to its ability to block prostaglandin synthesis by inhibiting cyclooxygenase 1 and 2. Prostaglandins have been shown to be mediators of certain kinds of intraocular inflammation.
Bromfenac Sodium Eye Prep is indicated for the treatment of postoperative inflammation and reduction of ocular pain in patients who have undergone cataract extraction. It is available as 0.07% or 0.09% solution.

Pharmacology
Bromfenac Sodium is an NSAID that inhibits COX-1 and COX-2 enzymes, reducing prostaglandin synthesis. Prostaglandins mediate intraocular inflammation, disruption of blood-aqueous humor barrier, vasodilation, increased vascular permeability, leukocytosis, and increased intraocular pressure.
Mechanism of action
Bromfenac Sodium inhibits COX-1 and COX-2 enzymes, reducing prostaglandin synthesis, which decreases the production of natural substances responsible for pain and inflammation in the eye.
Dosage
Adults (Postoperative inflammation):
0.07% or 0.09% solution: Instill 1 drop in the affected eye(s) once daily, starting 1 day before cataract surgery, continuing on the day of surgery, and through the first 14 days of the postoperative period.
Alternatively (0.09% solution): 1 drop twice daily (BID), beginning 24 hours after cataract surgery and continuing through the first 14 days of the postoperative period.
Prior to first use: If priming is required, follow the manufacturer's instructions for the dropper bottle.
Administration
For ophthalmic use only. Wash hands before use. Do not touch the dropper tip (to avoid contamination). Hold the dropper close to the eye without touching it and place the drop inside the lower eyelid. Apply pressure on the corner of the eye (close to the nose) for about 1 minute after instillation. If using contact lenses, remove them before using the drops and wait at least 15 minutes before reinserting. Use within 4 weeks of opening.
Missed dose
If you miss a dose, apply it as soon as you remember. Skip the missed dose if it is almost time for your next dose. Do not double the dose.
Side effects
Common side effects:
Abnormal sensation in eye, conjunctival hyperemia, eye irritation (including burning/stinging), eye pain, eye pruritus (itching), eye redness, headache, iritis.
Do not use after 4 weeks of opening the bottle.
Precautions & warnings
- All topical NSAIDs may slow or delay healing. Concomitant use with topical corticosteroids may increase the potential for healing problems.
- Use with caution in patients with known bleeding tendencies or who are receiving medications that may prolong bleeding time.
- May cause short-term blurring of vision after use; do not drive until vision is clear.
- Stinging sensation may occur for 1-2 minutes; notify your doctor if it persists longer.
- Caution during pregnancy and lactation.
Contraindications
- Known hypersensitivity to bromfenac sodium or any ingredients of the formulation
Drug interactions
- Concomitant use with topical corticosteroids: Increased potential for healing problems.
- No significant interactions with other medications are known.
Food interactions
No food-related restrictions (topical application).
Use in pregnancy
May be unsafe during pregnancy (Category C). Limited human studies available; animal studies have shown harmful effects on the developing baby. Your doctor will weigh benefits and risks before prescribing.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
Probably safe during breastfeeding (if prescribed). Limited human data suggests the drug does not pose significant risk to the baby.
Pediatric use
Safety and effectiveness in pediatric patients have not been established.
Geriatric use
No specific dose adjustment required for elderly, but caution advised.
Renal / hepatic impairment
No special precautions required for patients with liver or kidney impairment (minimal systemic absorption with topical application).
Overdose effects
Overuse may cause increased eye irritation or inflammation. Consult your doctor if symptoms occur.
Storage conditions
Store in a cool, dry place below 30°C, away from light. Keep out of reach of children. To prevent contamination, do not touch the dropper tip to eyelids, fingers, or other surfaces. Close the bottle tightly when not in use. Do not use after 4 weeks of first opening.
