Calcium Dobesilate 4% + Lidocaine 2% + Dexamethasone Acetate 0.025% Rectal Ointment is a combination preparation used for hemorrhoids and related anorectal conditions. Each gram contains Calcium Dobesilate BP 40 mg, Lidocaine Hydrochloride USP 20 mg, and Dexamethasone Acetate BP 0.25 mg.

Calcium Dobesilate acts on capillary walls by regulating their permeability and resistance and is effective on various signs of inflammation. It also has antithrombotic activity. Lidocaine hydrochloride is a local anesthetic that helps relieve pain. Dexamethasone Acetate is a topical corticosteroid with anti-inflammatory and antipruritic actions.

Pharmacology

Calcium Dobesilate acts on capillary walls by regulating their permeability and resistance and is effective on various signs of inflammation. It also has antithrombotic activity. Lidocaine hydrochloride is a local anesthetic that helps relieve pain. Dexamethasone Acetate is a topical corticosteroid with anti-inflammatory and antipruritic actions.

Mechanism of action

Calcium Dobesilate reduces capillary permeability and increases resistance, Lidocaine provides local anesthetic pain relief, and Dexamethasone reduces inflammation and itching.

Dosage

Adults: Apply 2-3 times daily. Apply in the morning and bedtime, and if possible after defecation.

Internal hemorrhoids: Use the applicator to insert as deep as possible into the anus and squeeze the tube to apply the ointment.

External hemorrhoids or anal pruritus: Apply a thin layer of the ointment several times a day.

Treatment duration: Generally a few days. Inform your doctor if symptoms have not improved or worsened after 1-2 weeks of treatment.

Renal insufficiency: Do not use for longer periods; avoid long-lasting treatments.

Administration

For rectal use only. Before use, remove the cap from the tube. Pierce the tube membrane by inverting the cap. Attach the applicator to the tube. Squeeze the tube to fill the applicator and lubricate the tip with ointment for smooth insertion. Insert the applicator gently and to full extent into the anus and squeeze once again from the end of the tube to force the required amount of ointment into the anal canal. Remove the applicator after application and discard it. Try not to have defecation for a couple of hours after internal application. For external use, squeeze a small amount of ointment onto the finger and apply around the outside of the anus. Wash hands thoroughly with warm water and soap after each application.

Missed dose

If you miss a dose, apply it as soon as you remember. Skip the missed dose if it is almost time for your next dose. Do not double the dose.

Side effects

Very rare side effects:
• Modifications of intestinal transit
• Temporary burning sensation
• Local pain
• Hypersensitivity reactions (with skin reactions and/or fever)

These reactions may be of allergic origin and treatment must be discontinued in such cases.

Precautions & warnings
  • In case of renal insufficiency, do not use for longer periods; avoid long-lasting treatments.
  • If allergic reactions (skin reactions, fever) occur, discontinue treatment and inform your doctor.
  • Caution during pregnancy and lactation.

Use in pregnancy

May be unsafe during pregnancy (Category C). Adequate human or animal studies are not available. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus.

Pregnancy Category: C — always consult a doctor before taking during pregnancy.

Use in lactation

Calcium dobesilate is excreted in maternal milk in low amounts after oral administration, but it is not known if this occurs with local use. Lidocaine and dexamethasone are excreted in maternal milk after topical administration. As a precaution, a decision should be made between discontinuing treatment or breastfeeding.

Pediatric use

Safety and efficacy in pediatric patients have not been established; consult your doctor.

Geriatric use

No specific dose adjustment required for elderly, but caution advised.

Renal / hepatic impairment
