Description
Carbonyl Iron + Folic Acid + Vitamin B Complex + Vitamin C + Zinc is a combination capsule used to supplement nutritional deficiencies during pregnancy and lactation. Each capsule contains Elemental Iron (as Carbonyl Iron) 50 mg, Zinc Sulphate 61.80 mg (equiv. Zinc 22.50 mg), Folic Acid 0.5 mg, Nicotinamide 10 mg, Riboflavin 2 mg, Pyridoxine HCl 1 mg, Ascorbic Acid 50 mg, and Thiamine Mononitrate 2 mg.
Carbonyl Iron has higher bioavailability, low toxicity, and better GI tolerance. Folic acid is required for normal neural tube development. Vitamin B Complex is required for the growth and development of unborn babies. Vitamin C plays a role in collagen structure in the fetal membrane. Zinc is a critical nutrient for fetal growth, central nervous system development, and better maternal and infant health.

Pharmacology
Carbonyl Iron has higher bioavailability and low toxicity. Folic acid is required for nucleoprotein synthesis and normal erythropoiesis. Vitamin B Complex plays a role in coenzyme A synthesis, lipid and carbohydrate metabolism. Vitamin C is necessary for collagen formation and tissue repair. Zinc is a cofactor in over 100 enzymes and supports a healthy immune system.
Mechanism of action
Iron supports hemoglobin formation and oxygen transport. Folic acid is required for red blood cell production and DNA synthesis. Vitamin B Complex maintains energy metabolism and nervous system function. Vitamin C acts as an antioxidant and enhances iron absorption. Zinc acts as a cofactor in enzymatic reactions and strengthens the immune system.
Dosage
Adults: One capsule daily before food. In more severe cases, 2 capsules a day may be required, or as directed by the physician.
Children & Adolescents: May be used upon consultation with a doctor.
Note: For maximum absorption, take on an empty stomach; may take with or after meals to minimize GI irritation.

Missed dose
If you miss a dose, take it as soon as you remember. Skip the missed dose if it is almost time for your next dose. Do not double the dose.
Side effects
Common side effects: Nausea, vomiting, constipation, or diarrhea (from oral iron preparations).
Allergic reactions: Allergic sensitization may occur rarely.
Oral iron preparations may cause constipation, particularly in older patients.
Precautions & warnings
- Caution in patients who may develop iron overload (hemochromatosis, hemolytic anemia, red cell aplasia).
- Iron chelates with tetracycline, impairing absorption; maintain at least 2-3 hours gap.
- Antacids decrease iron absorption; maintain at least 2 hours gap.
- Avoid use during the first trimester of pregnancy if possible; use during second and third trimesters under medical supervision.
- Present in breast milk; use with caution during lactation.
Contraindications
- Known hypersensitivity to any of the ingredients
- Hemolytic anemia (iron therapy is contraindicated)
Drug interactions
- Antacids: May decrease carbonyl iron absorption (maintain at least 2 hours gap).
- Tetracyclines, quinolone antibiotics: Iron reduces absorption (maintain at least 2-3 hours gap).
- Levodopa, levothyroxine, methyldopa, penicillamine: Iron may reduce absorption.
- Antiepileptics (phenobarbital, phenytoin, primidone): Folic acid may reduce plasma concentrations.
- Oral contraceptives, anti-TB drugs, alcohol, methotrexate, pyrimethamine, trimethoprim, sulphonamides: May decrease serum folate concentrations.
- Vitamin C: May interact with deferoxamine, hormonal contraceptives, fluphenazine, warfarin, salicylates, disulfiram, mexiletine, vitamin B12.
Food interactions
For maximum absorption, take on an empty stomach. Calcium-rich foods (milk, dairy) and caffeine may reduce iron absorption; maintain at least 1-2 hours gap.
Use in pregnancy
Avoid use during the first trimester of pregnancy if possible; may be used during the second and third trimesters under medical supervision. Iron deficiency must be confirmed before use in the first trimester.
Use in lactation
May be used during lactation (if prescribed); present in breast milk.
Pediatric use
May be used in children and adolescents upon consultation with a doctor.
Geriatric use
Elderly patients have higher risk of constipation; ensure adequate fluid and fiber intake.
Renal / hepatic impairment
No special precautions required for patients with liver or kidney impairment, but consult your doctor.
Overdose effects
Overdose symptoms: Decreased energy, nausea, abdominal pain, tarry stool, weak rapid pulse, fever, coma, seizures. Consult your doctor immediately if overdose is suspected.
