Cinchocaine HCI + Hydrocortisone + Framycetin Sulphate + Esculin (Rectal prep)

Cinchocaine HCI + Hydrocortisone + Framycetin Sulphate + Esculin is a combination medicine used for the treatment of internal and external haemorrhoids, post-partum haemorrhoids, anal pruritus, peri-anal eczema, anal fissures, proctitis, and post-haemorrhoidectomy application to relieve pain and discomfort. Cinchocaine Hydrochloride is a local anaesthetic that prevents or relieves severe pain. Hydrocortisone is a corticosteroid that acts as a decongestant, anti-inflammatory and anti-pruritic agent, eliminating itching, inflammation and mucous discharge. Framycetin Sulphate is a broad-spectrum antibiotic that eradicates infections in the anorectal area. Esculin has a skin protective action.

The local anaesthetic Cinchocaine Hydrochloride prevents or relieves the severe pain sometimes encountered in strangulated haemorrhoids, fissures and perianal haematomata, whilst the corticosteroid (Hydrocortisone) acts as a decongestant, anti-inflammatory and anti-pruritic agent and by so doing eliminates itching, inflammation and mucous discharge. The broad spectrum antibiotic, Framycetin Sulphate, will eradicate most infections which may already be present or arise in lesions of the anorectal area and Esculin has a skin protective action.

Cinchocaine prevents or diminishes nerve impulse conduction near the site of administration. Hydrocortisone suppresses migration of polymorphonuclear leukocytes and reverses increased capillary permeability. Framycetin Sulphate inhibits bacterial protein synthesis. Esculin improves skin vasculature.

Adult Dose (Ointment): Apply a small quantity onto the affected area in the morning and evening and after each stool. For deep application, attach applicator/cannula to tube, insert to full extent and squeeze tube gently from lower end whilst withdrawing.

Adult Dose (Suppository): Insert 1 suppository in the morning and evening and after each stool.

Apply the ointment in a small quantity with the finger, on the painful or pruritic area, morning and evening and after each stool. For deep application, attach applicator/cannula to tube, insert to full extent and squeeze tube gently from lower end whilst withdrawing. Insert 1 suppository in the morning and evening and after each stool.

If you miss a dose of this medicine, use it as soon as you remember. However, if it is almost time for your next application, skip the missed dose and go back to your regular schedule. Do not double the dose.

• Paraesthesias
• Dizziness
• Tinnitus
• Blurred vision
• Nausea, vomiting
• Muscle twitching, tremors
• Convulsions
• Hypotension
• Bradycardia
• Arrhythmias
• Cardiac arrest

Long-term use: Skin atrophy (thinning), loss of elasticity, telangiectasia, ecchymoses, HPA axis suppression, growth retardation (children), Cushingoid state.

• Avoid application for prolonged periods and to extensive areas.
• Discontinue use if sensitization occurs.
• Specific measures against infections, allergy, and other causal factors must not be neglected.
• Occlusive dressings increase the risk of side effects.
• Not recommended for use in children.
• Caution during pregnancy and lactation.

• Hypersensitivity to any of the ingredients
• Application to inflamed or infected tissues or to damaged skin mucosa
• Instillation into the middle ear
• Herpes simplex, vaccinia or varicella, or tuberculous infection of the anal region
• Pregnancy

No food interactions have been found or established.

The safe use of topical corticosteroids during pregnancy has not been fully established. Should not be used unnecessarily on extended areas, in large amounts or for prolonged periods during pregnancy. Animal studies have shown teratogenic effects with topical corticosteroids.

Limited information is available regarding the use of this medicine during breastfeeding. Consult your doctor before use.

Not recommended for use in children. Children are at higher risk of HPA axis suppression, growth retardation, and Cushingoid state.

Limited information is available regarding dose adjustment for elderly patients.

For topical use, no dose adjustment is required in renal or hepatic impairment.

Information on overdosage is not available. Consult a physician if overdose is suspected.
