Description
Deucravacitinib is an inhibitor of tyrosine kinase 2 (TYK2). TYK2 is a member of the Janus kinase (JAK) family. Deucravacitinib binds to the regulatory domain of TYK2, stabilizing an inhibitory interaction between the regulatory and the catalytic domains of the enzyme. This results in allosteric inhibition of receptor-mediated activation of TYK2 and its downstream activation of STATs.

Pharmacology
Deucravacitinib is an inhibitor of tyrosine kinase 2 (TYK2). TYK2 is a member of the Janus kinase (JAK) family. Deucravacitinib binds to the regulatory domain of TYK2, stabilizing an inhibitory interaction between the regulatory and the catalytic domains of the enzyme. This results in allosteric inhibition of receptor-mediated activation of TYK2 and its downstream activation of STATs.
Mechanism of action
Deucravacitinib binds to the regulatory domain of TYK2, inhibiting enzyme activation, which blocks cytokine pathways and reduces inflammation in psoriasis.
Dosage
Adult: 6 mg once daily orally, with or without food. Do not crush, cut, or chew the tablets.
Severe hepatic impairment (Child-Pugh C): Not recommended.
Administration
Take once daily at approximately the same time each day, with or without food. Swallow the tablet whole, do not chew, cut, or crush.
Missed dose
If you miss a dose of Deucravacitinib, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose. Do not double the dose.
Side effects
Common: Upper respiratory infections (common cold, sore throat, sinus infection), herpes simplex (cold sores), aphthous ulcers (canker sores), folliculitis, acne.
Serious: Allergic reactions (angioedema, breathing difficulty, rash, hives).
Precautions & warnings
Avoid use in active or serious infection; evaluate for TB prior to treatment; avoid live vaccines; monitor liver enzymes and triglycerides; discontinue if hypersensitivity occurs; avoid use in severe hepatic impairment.
Contraindications
Hypersensitivity.
Drug interactions
Cyclosporine, fluvoxamine, ritonavir, diflunisal, pyrimethamine, famotidine, rabeprazole: No clinically significant differences in deucravacitinib pharmacokinetics were observed.
Rosuvastatin, methotrexate, mycophenolate mofetil, oral contraceptives: No clinically significant differences in pharmacokinetics were observed.
Food interactions
Can be taken with or without food.
Use in pregnancy
Available data on Deucravacitinib use during pregnancy are insufficient. No effect on embryo-fetal development was seen in animal reproductive studies.
Pregnancy Category: N/A — always consult a doctor before taking during pregnancy.
Use in lactation
There are no data on the presence of Deucravacitinib in human milk. Consider developmental and health benefits of breastfeeding along with the mother's clinical need.
Pediatric use
Safety and effectiveness in pediatric patients have not been established.
Geriatric use
No overall differences in effectiveness have been observed between patients 65 years of age and older and younger adult patients.
Renal / hepatic impairment
Renal impairment: No dose adjustment is recommended in patients with mild, moderate, or severe renal impairment or ESRD on dialysis.
Hepatic impairment: Mild to moderate: No dosage adjustment needed; Severe (Child-Pugh C): Not recommended.
Overdose effects
Treatment of overdose should be symptomatic.
