Description

Pharmacology

Diclofenac is an NSAID that inhibits cyclooxygenase, reducing prostaglandin synthesis. Lidocaine is an amide-type local anesthetic that stabilizes the neuronal membrane and inhibits Na+ ion movement.

Mechanism of action

Diclofenac reduces pain and inflammation by inhibiting prostaglandin synthesis; Lidocaine provides local pain relief by blocking nerve conduction.

Dosage

Adult (IM): 75 mg once daily (may increase to 75 mg bid in severe conditions); max period: 2 days.
Adult (IV - Postoperative pain): 75 mg infusion over 30-120 min or bolus, may repeat after 4-6 hours; max 2 days.
Adult (Renal colic): 75 mg (may repeat once after 30 min); max 150 mg/day.
Children (Juvenile chronic arthritis): 1-3 mg/kg/day.
Elderly: Lowest effective dose.

Administration

Administered as intramuscular injection (gluteal muscle) or intravenous infusion by healthcare professionals.

Missed dose

This medicine is given by healthcare professionals. You cannot miss a dose yourself.

Side effects

Common: GI discomfort, nausea, diarrhea, headache, injection site reactions.
Rare: Peptic ulcer, haematemesis, melena, blood dyscrasia, anaphylactoid reactions.
Lidocaine-related (CNS): Drowsiness, dizziness, confusion, lightheadedness.

Precautions & warnings

Caution in GI ulceration, haematemesis, melena, ulcerative colitis, asthma, Crohn's disease, bleeding diathesis, severe hepatic/cardiac/renal insufficiency, hypertension.

Contraindications

Active or suspected peptic ulcer or GI bleeding; aspirin or other NSAID-induced asthma, urticaria or acute rhinitis; hypersensitivity to amide-type local anesthetics; Adams-Stokes syndrome; SA, AV or intraventricular heart block.

Drug interactions

Lithium, Digoxin: Increased plasma concentrations.
Anticoagulants (warfarin): Increased risk of hemorrhage.
Cyclosporin: Increased risk of nephrotoxicity.
Methotrexate: Risk of serious toxicity.
Quinolone antibiotics: Convulsions may occur.
Other NSAIDs and steroids: Increased frequency of unwanted effects.
Diuretics: Activity may be inhibited.

Food interactions

Can be used with or without food.

Use in pregnancy

Pregnancy Category C; avoid in late pregnancy (may cause premature closure of ductus arteriosus); Category D if >30 weeks of gestation.

Pregnancy Category: C — always consult a doctor before taking during pregnancy.

Use in lactation

Diclofenac is excreted in breast milk in small quantities; use is not recommended.

Pediatric use

Juvenile chronic arthritis: 1-3 mg/kg/day may be used.

Geriatric use

Elderly or debilitated patients: lowest effective dose.

Renal / hepatic impairment

Caution in renal impairment; dose adjustment may be needed.

Overdose effects

Treatment of overdose is symptomatic.
