Dydrogesterone is a progestin (female hormone) used for female infertility, menstrual problems, premenstrual syndrome (PMS), endometriosis, and prevention of threatened miscarriage. It regulates the healthy growth and shedding of the womb lining.

Description

Dydrogesterone is an orally-active progestogen that produces a complete secretory endometrium in an estrogen-primed uterus. It is indicated for threatened miscarriage, habitual miscarriage, dysmenorrhoea, endometriosis, secondary amenorrhoea, irregular cycles, dysfunctional uterine bleeding, infertility due to luteal insufficiency, luteal support in ART, and HRT. It has no estrogenic, androgenic, thermogenic, anabolic or corticoid activity. Common side effects include headache, nausea, and breast pain. Contraindicated in severe hepatic dysfunction, thromboembolic disorders, and progestogen-dependent neoplasms.

Dydrogesterone is an orally-active progestogen which produces a complete secretory endometrium in an estrogen-primed uterus, providing protection against estrogen-induced endometrial hyperplasia and carcinogenesis. It has no estrogenic, androgenic, thermogenic, anabolic or corticoid activity.

Mechanism of action

Dydrogesterone is a progestogen structurally related to progesterone. Unlike progesterone, it does not induce an increase in temperature nor inhibit ovulation and may be preferred over other progestational agents when contraceptive effect is not required.

Dosage

Adult Dose (Oral):

  • Endometriosis: 10 mg bid-tid cyclically or continuously.
  • Habitual miscarriage: 10 mg bid cyclically until conception, then continuously until week 20 of pregnancy.
  • Menstrual disorders: 10 mg bid cyclically.
  • Threatened miscarriage: Initially 40 mg, then 10 mg or more every 8 hours for 1 week after symptoms relieved.
  • Infertility: 10 mg bid.
  • HRT: 10 mg 1-2 times daily cyclically or 5 mg daily.
Administration

Oral tablet. Take in the dose and duration as advised by your doctor. Swallow whole. Do not chew, crush or break. May be taken with or without food, but better to take at a fixed time.

Missed dose

If you miss a dose of Dydrogesterone, take it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular schedule.

Precautions & warnings
  • Dydrogesterone is used for menstrual disorders and to maintain a healthy womb lining for pregnancy.
  • In case of repeated miscarriages, seek regular prenatal care.
  • Avoid smoking, alcohol and limit caffeine intake (may lead to miscarriage).
  • It may cause bleeding or spotting between periods; inform doctor if frequent.
  • Inform doctor if you have history of liver disease or depression.
  • Etiology of abnormal bleeding should be clarified before treatment.

Drug interactions

Carbamazepine, griseofulvin, phenobarbital, rifampicin enhances clearance of progestogens.

Food interactions

It is not known whether it is safe to consume alcohol with Dydrogesterone. Please consult your doctor.

Use in pregnancy

No evidence of harmful effect of Dydrogesterone during pregnancy. Consult your doctor.

Use in lactation

Dydrogesterone is safe during breastfeeding. Human studies suggest drug does not pass into breastmilk in significant amount and is not harmful to the baby.

Pediatric use

Safety and efficacy in adolescents aged 12-18 years have not been established.

Geriatric use

Limited information available regarding dose adjustment for elderly patients.

Renal / hepatic impairment

Renal impairment: Limited information available on use in kidney disease. Consult your doctor.

Hepatic impairment: Use with caution. Dose adjustment may be needed. Not recommended in severe liver disease.

Overdose effects

Oral doses up to 360 mg/day were well tolerated. No ill effects from overdose reported.
