Elemental Iron (Iron iii Hydroxide Sucrose Complex)

Elemental Iron (Iron iii Hydroxide Sucrose Complex) is an anti-anemic medication that replenishes iron stores in the body. Iron is vital for the formation of new red blood cells and hemoglobin. It is administered as injection or infusion by a doctor or nurse. Used for iron-deficiency anemia and anemia due to chronic kidney disease. Dosage: 100-400 mg IV depending on CKD type. Common side effects include injection site reactions, nausea, and dark colored stools. Contraindicated in iron overload conditions and anemia not caused by iron deficiency.

Iron is essential for hemoglobin formation and effective erythropoiesis; also serves as a cofactor of several essential enzymes. Replacement of iron stores found in hemoglobin, myoglobin, and enzymes; works to transport oxygen via hemoglobin.

Adult Dose:

• Hemodialysis-dependent CKD: 100 mg elemental iron IV (injection or infusion over 2-5 min) per dialysis session, not to exceed total 1000 mg divided in 3 doses/week.
• Non-dialysis-dependent CKD: 200 mg IV injection for 5 doses over 14 days (cumulative 1000 mg).
• Peritoneal dialysis-dependent CKD: 300 mg IV infusion (1.5 hr) for 2 doses 14 days apart, THEN 400 mg IV infusion (2.5 hr) 14 days later.

Child Dose: 0.5 mg/kg IV q2-4 weeks for 12 weeks; not to exceed 100 mg/dose.

Administered as intravenous injection or infusion by a doctor or nurse. Do not self-administer.

Common Side Effects:

• Injection site reactions (pain, swelling, redness)
• Nausea
• Dark colored stool
• Vomiting
• Constipation

• Elemental Iron is given as a slow injection or infusion under doctor supervision.
• Take as per dose and duration suggested by the doctor.
• Avoid drinking alcohol (may worsen side effects).
• Regular blood tests may be needed to check blood cells and iron levels.
• May cause lowering of blood pressure; continuous monitoring required.
• Inform doctor if pregnant, planning pregnancy or breastfeeding.

• Evidence of iron overload (e.g., haemochromatosis, thalassemia, haemosiderosis)
• Known hypersensitivity to iron preparations or any components
• Anemia not caused by iron deficiency

May reduce serum levels of cefdinir; stools may appear red. May reduce serum levels of eltrombopag. May decrease absorption of phosphate supplements. Concurrent use with trientine may reduce serum levels of iron sucrose and/or trientine. Dimercaprol may increase nephrotoxic effect of iron salts.

It is unsafe to consume alcohol with Elemental Iron.

Generally considered safe during pregnancy. Animal studies have shown low or no adverse effects; however, limited human studies.

Safe during breastfeeding. Human studies suggest the drug does not pass into breastmilk in significant amount and is not harmful to the baby.

Under 2 years: Safety and efficacy not established.

Limited information available regarding dose adjustment for elderly patients.

Renal impairment: Use with caution. Dose adjustment may be needed.

Hepatic impairment: Use with caution. Dose adjustment may be needed.

Overdose may cause iron overload, nausea, vomiting, diarrhea and allergic reactions. Provide symptomatic treatment.

Store in a cool and dry place. Keep out of reach of children.
