Elemental Iron (Iron iii Hydroxide Sucrose Complex) is an iron replacement product used for the treatment of iron-deficiency anemia and anemia associated with chronic kidney disease.
Indications
- Iron deficiency anemia
- Anemia due to chronic kidney disease
Dosage
Adult Dose:
- Hemodialysis-dependent CKD: 100 mg elemental iron IV (injection or infusion over 2-5 min) per dialysis session, not to exceed total 1000 mg divided in 3 doses/week.
- Non-dialysis-dependent CKD: 200 mg IV injection for 5 doses over 14 days (cumulative 1000 mg).
- Peritoneal dialysis-dependent CKD: 300 mg IV infusion (1.5 hr) for 2 doses 14 days apart, THEN 400 mg IV infusion (2.5 hr) 14 days later.
Child Dose: 0.5 mg/kg IV q2-4 weeks for 12 weeks; not to exceed 100 mg/dose.
Side effects
Common Side Effects:
- Injection site reactions (pain, swelling, redness)
- Nausea
- Dark colored stool
- Vomiting
- Constipation
Precautions & warnings
- Elemental Iron is given as a slow injection or infusion under doctor supervision.
- Take as per dose and duration suggested by the doctor.
- Avoid drinking alcohol (may worsen side effects).
- Regular blood tests may be needed to check blood cells and iron levels.
- May cause lowering of blood pressure; continuous monitoring required.
- Inform doctor if pregnant, planning pregnancy or breastfeeding.
