Elobixibat is a medicine used for the treatment of chronic idiopathic constipation (except for constipation associated with organic diseases). It inhibits ileal bile acid transporter (IBAT) to increase bile acid in the intestinal lumen and enhance colonic motility.
Description
Elobixibat inhibits bile acid reabsorption via ileal bile acid transporter (IBAT) expressed on the epithelial cells of the terminal ileum, increasing the amount of bile acid passing into the large intestinal lumen. Bile acid promotes secretion of water and electrolytes into the large intestinal lumen and enhances colonic motility, inducing therapeutic effect on constipation. The usual adult dose is 10 mg once daily before meal, maximum 15 mg/day. Common side effects include abdominal pain, diarrhea, nausea, abdominal discomfort, soft faeces, abdominal distension and liver function test abnormalities.

Pharmacology
Elobixibat inhibits bile acid reabsorption via IBAT expressed on the epithelial cells of the terminal ileum, increasing the amount of bile acid passing into the large intestinal lumen. Bile acid promotes secretion of water and electrolytes and enhances colonic motility.
Mechanism of action
Elobixibat inhibits IBAT to increase bile acid in the intestine, which promotes water and electrolyte secretion and colonic motility, relieving constipation.
Dosage
Adult Dose: 10 mg once daily before meal. Dosage may be adjusted based on patient's symptoms but must not exceed 15 mg per day.
Elderly Dose: Caution should be exercised, such as reducing the dose, due to reduced physiological functions.
Administration
Oral tablet. Take once daily before meal.
Missed dose
If you miss a dose of Elobixibat, take it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular schedule.

Precautions & warnings
- Elobixibat may cause abdominal pain or diarrhea; dose reduction, drug withdrawal or discontinuation should be considered based on symptoms.
- May not achieve expected efficacy in serious liver disorder (due to biliary obstruction or reduced bile acid secretion).
- Regular evaluation of need for continuing treatment to avoid aimless administration.
Contraindications
- Hypersensitivity to active substance or any excipient
- Intestinal obstruction associated with tumor or hernia or suspicion
Drug interactions
Bile acid preparations (ursodeoxycholic acid, chenodeoxycholic acid): Elobixibat's IBAT inhibition may interfere with reabsorption.
Aluminum-containing antacids, sucralfate, aldioxa: These drugs absorb bile acids and may attenuate Elobixibat effect.
Cholestyramine, colestimide: These drugs absorb bile acids and may attenuate Elobixibat effect.
Digoxin, dabigatran etexilate: Blood levels may elevate due to P-glycoprotein inhibition.
Midazolam: Blood level may decrease, reducing midazolam effect.
Food interactions

Use in pregnancy
Should be used in pregnant women only if expected therapeutic benefits outweigh possible risks.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
Lactating women should avoid Elobixibat. If treatment is essential, breastfeeding must be discontinued during treatment.
Pediatric use
Safety not established in low-birth-weight infants, neonates, nursing infants or pediatric patients (no clinical experience).
Geriatric use
Elderly generally have reduced physiological functions; caution such as dose reduction should be exercised.
Renal / hepatic impairment
Renal impairment: Use with caution in kidney disease.
Hepatic impairment: May not achieve expected efficacy in serious liver disorder. Caution required.
Overdose effects
No data on overdose. Do not exceed indicated dosing. Actively monitor for timely response.

Do not store above 25°C. Protect from light. Keep out of reach of children.
