Flumazenil

Flumazenil is used to reverses the effect of sedatives that are used before surgery or other medical procedures. It helps to regain consciousness or wake up the patient after the surgical procedures. It is given under the supervision of your doctor.

• Benzodiazepine Overdose: Used for reversal of centrally sedative effects of benzodiazepines in anaesthesia and intensive care.
• Diagnosis of Unconsciousness: Used to differentiate between benzodiazepine involvement and other causes in unconsciousness of unknown origin.

Adult Dose (IV):

• Anaesthesia: Initial: 0.2 mg IV over 15 seconds; if desired consciousness not obtained within 60 seconds, repeat 0.1 mg (at 60-second intervals, max 1 mg); usual dose: 0.3-0.6 mg.
• Intensive Care/Unconsciousness: Initial: 0.3 mg IV; if desired consciousness not obtained within 60 seconds, repeat (max 2 mg); for re-sedation: 0.1-0.4 mg/hr IV infusion.

Child Dose (>1 year): Initial: 0.01 mg/kg (max 0.2 mg) IV over 15 seconds; repeat if needed after 45 seconds (max 0.05 mg/kg or 1 mg, whichever is lower).

For IV use only; should be administered by an anaesthesiologist or experienced physician.

Common Side Effects: Anxiety, Insomnia, Allergic reaction, Sleepiness, Palpitations, Fear.
Serious Side Effects: Seizures (in epileptic patients or severe hepatic impairment), withdrawal symptoms (in long-term benzodiazepine users).

Precautions: It is not known whether Flumazenil alters the ability to drive; do not drive if you experience any symptoms. Particular caution in mixed-substance overdose. Not recommended in epileptic patients who have been receiving benzodiazepine treatment for a prolonged period. Caution in patients with head injury. Avoid rapid injection. Not recommended for treatment of benzodiazepine dependence or protracted abstinence syndromes.

Contraindications: Hypersensitivity; Patients who have been given a benzodiazepine for control of a potentially life-threatening condition (e.g., control of intracranial pressure or status epilepticus).

Drug Interactions: Benzodiazepines - Competitively blocks central effects; Non-benzodiazepine agonists (zopiclone, triazolopyridazines) - Effects are also blocked; Ethanol - No pharmacokinetic interaction.

Food Interactions: Caution is advised when consuming alcohol with Flumazenil; consult your doctor.

Use in Pregnancy: Flumazenil may be unsafe; animal studies have shown harmful effects; parenteral administration in emergencies is not contraindicated.

Use in Lactation: Flumazenil is probably safe; limited data suggests no significant risk to the baby.

Pediatric Use: Dose for children >1 year provided above; limited experience in children under 1 year, use with caution.

Geriatric Use: No specific dosage adjustment is necessary for elderly patients.

Renal Impairment: Safe to use in patients with kidney disease; no dose adjustment recommended.
Hepatic Impairment: Use with caution; dose adjustment may be needed (extended observation period may be required).

Overdose: Limited information on overdose; well tolerated even at doses exceeding recommended; symptomatic treatment.

Storage: Keep below 30 degree C, away from light and moisture. Keep out of reach of children.
