Flumazenil is an antidote used to reverse the sedative effects of benzodiazepines given during surgery.

Indications
  • Benzodiazepine Overdose: Used for reversal of centrally sedative effects of benzodiazepines in anaesthesia and intensive care.
  • Diagnosis of Unconsciousness: Used to differentiate between benzodiazepine involvement and other causes in unconsciousness of unknown origin.
Dosage

Adult Dose (IV):

  • Anaesthesia: Initial: 0.2 mg IV over 15 seconds; if desired consciousness not obtained within 60 seconds, repeat 0.1 mg (at 60-second intervals, max 1 mg); usual dose: 0.3-0.6 mg.
  • Intensive Care/Unconsciousness: Initial: 0.3 mg IV; if desired consciousness not obtained within 60 seconds, repeat (max 2 mg); for re-sedation: 0.1-0.4 mg/hr IV infusion.

Child Dose (>1 year): Initial: 0.01 mg/kg (max 0.2 mg) IV over 15 seconds; repeat if needed after 45 seconds (max 0.05 mg/kg or 1 mg, whichever is lower).

Side effects

Common Side Effects: Anxiety, Insomnia, Allergic reaction, Sleepiness, Palpitations, Fear.
Serious Side Effects: Seizures (in epileptic patients or severe hepatic impairment), withdrawal symptoms (in long-term benzodiazepine users).

Precautions & warnings

Precautions: It is not known whether Flumazenil alters the ability to drive; do not drive if you experience any symptoms. Particular caution in mixed-substance overdose. Not recommended in epileptic patients who have been receiving benzodiazepine treatment for a prolonged period. Caution in patients with head injury. Avoid rapid injection. Not recommended for treatment of benzodiazepine dependence or protracted abstinence syndromes.
