Description
Hydrocortisone + Lidocaine ano-rectal cream is used for anti-inflammatory and anesthetic relief. Hydrocortisone acetate provides relief of inflammatory and pruritic manifestations of corticosteroid-responsive dermatoses. Lidocaine is an amide-type local anesthetic that has analgesic effects and acts via reversible block of impulses from nerve fibres.

Pharmacology
Hydrocortisone is a corticosteroid used for its anti-inflammatory and immunosuppressive effects. Its anti-inflammatory action is due to the suppression of migration of polymorphonuclear leukocytes and reversal of increased capillary permeability. Lidocaine is an amide type local anesthesia that stabilises the neuronal membrane and inhibits Na ion movements necessary for conduction of impulses.
Mechanism of action
Hydrocortisone reduces inflammation and itching; Lidocaine relieves pain by blocking nerve impulses.
Dosage
Adult Dose:
- Hemorrhoids, Anal fissures, Pruritus ani: Apply BID to areas of discomfort and anal opening
- Applicator tip should be gently inserted into the anal area (areas of discomfort and anal opening)
- Applicator and tube should not completely insert in the anus or rectum
Child Dose: Safety and efficacy not established
Administration
For external use only; apply to affected area(s) twice daily or as directed by physician; applicator tip should be gently inserted into the anal area.
Missed dose
If you miss a dose, apply it as soon as possible; if it is almost time for your next dose, skip the missed dose.

Precautions & warnings
Precautions:
- If extensive areas are treated, possibility of systemic absorption exists
- Systemic absorption of topical steroids may cause hypothalamic-pituitary-adrenal (HPA) axis suppression
- If irritation or sensitivity occurs or infection appears, discontinue use
- Caution with impaired hepatic function and patients taking class I antiarrhythmics
- Avoid using for prolonged periods or with occlusive dressings
Contraindications
Contraindications:
- Patients with a history of hypersensitivity to any of its ingredients
Drug interactions
Drug Interactions:
- Use with caution with dental injection anaesthesia, other local anaesthetics or agents structurally related to amide-type local anaesthetics (e.g., antiarrhythmic drugs mexiletine)
- Class III antiarrhythmic drugs (e.g., amiodarone): Cardiac effects can be additive
- Drugs that inhibit lidocaine metabolism (e.g., cimetidine or β-blockers) may cause potentially toxic plasma concentrations when lidocaine is given in repeated high doses over a long time period
