Isosorbide Dinitrate

Isosorbide Dinitrate is used to prevent chest pain (angina attacks). It works by relaxing and widening blood vessels so blood can flow more easily to the heart. These tablets will not treat an angina attack that has already begun. Isosorbide Dinitrate must be taken in the dose and duration as advised by the doctor. Place the tablet under the tongue or between the cheek and gum, and let it dissolve. It is usually taken as needed, either 5 to 10 minutes before physical activities or at the first sign of angina. You should take the medicine as it is prescribed for you and keep taking it, even if you feel well. If you stop taking it suddenly your risk of an angina attack will increase. Lifestyle changes such as diet, exercise, and not smoking may help this drug work better. You can develop tolerance to this medicine over time, which means that the same dose becomes less effective. You have to stick to a really specific dosing schedule to prevent this from happening. The most common side effects of this medicine include headache and lightheadedness. These may disappear once you have got used to the medicine. If you are worried, or the symptoms do not go away, let your doctor know. Some of these can be reduced by taking painkillers, drinking plenty of fluids, and avoiding alcohol. Alcohol may increase some side effects (dizziness, drowsiness, feeling light-headed, or fainting). Medical tests (such as blood pressure) may be required to monitor your progress and check for side effects. Isosorbide Dinitrate is a nitrate. It works by relaxing the blood vessels which decreases the oxygen demand of the heart and reduces its workload, thereby preventing/treating attacks of angina (chest pain).

Isosorbide dinitrate relaxes vascular smooth muscles by stimulating cyclic-GMP. It decreases left ventricular pressure (preload) and arterial resistance (afterload).

Adult Dose: Angina Pectoris Immediate release: 5-20 mg PO q8-12hr initially; maintenance: 10-40 mg PO q8-12hr

Place the tablet under the tongue or between the cheek and gum, and let it dissolve. Do not chew, crush, or swallow the tablet. Isosorbide Dinitrate may be taken with or without food, but it is better to take it at a fixed time.

If you miss a dose of Isosorbide Dinitrate, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and go back to your regular schedule. Do not double the dose.

[SAFE] Isosorbide Dinitrate does not usually affect your ability to drive. Isosorbide Dinitrate may cause dizziness or sleepiness. Do not drive or do anything that requires concentration until you know how it affects you. Do not consume alcohol while taking Isosorbide Dinitrate as it may cause increased dizziness. It may cause headaches which usually go away after a few days of continued treatment. Inform your doctor if it persists. You may develop tolerance to Isosorbide Dinitrate with time, meaning that the same dose may be less effective next time. Follow the prescribed dosing schedule to avoid this. Stop taking medications (prescribed for erectile dysfunction) like sildenafil or tadalafil while on treatment with Isosorbide Dinitrate as it may cause low blood pressure.

Severe hypotension or anaemia, hypovolaemia, heart failure due to obstruction, or raised intracranial pressure due to head trauma or cerebral haemorrhage.

Increased hypotensive effects with alcohol or vasodilators. Marked orthostatic hypotension may occur when used with calcium channel blockers. Vasodilatory effect may be reduced with dihydroergotamine. Ergotamine effects may be enhanced. Reduced effectiveness of sublingual form with disopyramide. Potentially Fatal: Significant hypotension may occur with phosphodiesterase-5 inhibitors.

[UNSAFE] Consuming alcohol while taking Isosorbide Dinitrate may cause symptoms such as flushing, increased heart beat, nausea, thirst, chest pain and low blood pressure (Disulfiram reaction).

[CONSULT YOUR DOCTOR] Isosorbide Dinitrate may be unsafe to use during pregnancy. Although there are limited studies in humans, animal studies have shown harmful effects on the developing baby. Your doctor will weigh the benefits and any potential risks before prescribing it to you. Please consult your doctor.

[SAFE IF PRESCRIBED] Isosorbide Dinitrate is probably safe to use during breastfeeding. Limited human data suggests that the drug does not represent any significant risk to the baby.

[SAFE IF PRESCRIBED] Isosorbide Dinitrate is safe to use in patients with kidney disease. No dose adjustment of Isosorbide Dinitrate is recommended. However, inform your doctor if you have an underlying kidney disease. [SAFE IF PRESCRIBED] Isosorbide Dinitrate is probably safe to use in patients with liver disease.
