Lomefloxacin is an antibiotic used in the treatment of bacterial infections of the urinary tract, nose, throat, skin, soft tissues, and lungs (pneumonia).
Description
Lomefloxacin is a difluorinated quinolone that interferes with bacterial DNA-related processes including replication, transcription, DNA repair, recombination, and supercoiling by inhibiting DNA gyrase (topoisomerase II).
Indications
- Acute bacterial exacerbation of chronic bronchitis
- Uncomplicated urinary tract infections (E. coli: 3 days; others: 10 days)
- Complicated urinary tract infections (E. coli, K. pneumoniae, P. mirabilis, P. aeruginosa: 14 days)
- Prophylaxis of surgical infections
- Bacterial conjunctivitis, blepharitis, keratitis, corneal ulcer (eye drops)

Pharmacology
Lomefloxacin inhibits bacterial DNA gyrase (topoisomerase II) to prevent DNA replication, transcription, repair, and recombination.
Mechanism of action
Inhibits DNA gyrase to prevent bacterial DNA replication and repair, killing bacterial cells.
Dosage
Adult Dose (Oral):
- Chronic bronchitis: 400 mg once daily, 10 days
- Uncomplicated UTI: 400 mg once daily (E. coli: 3 days; others: 10 days)
- Complicated UTI: 400 mg once daily, 14 days
- Surgical prophylaxis: 400 mg single dose 1-6 hours before procedure
Renal Impairment: CrCl 10-40: Initial 400 mg/day, Maintenance 200 mg/day
Administration
- Swallow tablet whole; do not chew, crush or break
- May be taken with or without food, but at a fixed time is preferable
- Maintain 2-hour interval with aluminum or magnesium-containing antacids
Missed dose
If you miss a dose, take it as soon as possible. If it's almost time for your next dose, skip the missed dose and go back to your regular schedule. Do not double the dose.

Precautions & warnings
- Caution when driving or activities requiring concentration (may cause dizziness)
- Discontinue if tendon pain, swelling, or rupture signs occur
- Inform doctor if diarrhea persists or blood in stools
- Consult doctor during pregnancy and lactation
Contraindications
- Hypersensitivity to Lomefloxacin or other quinolones
- Children under 18 years
- Pregnancy and lactation
Drug interactions
- Sucralfate, aluminum/magnesium antacids, iron supplements reduce absorption
- Cimetidine increases serum levels
- Probenecid reduces renal elimination
- Increased risk of ventricular arrhythmias with Class IA/III antiarrhythmics (potentially fatal)
Food interactions
Alcohol consumption with Lomefloxacin is safe.
Use in pregnancy
Use in Pregnancy: Lomefloxacin may be unsafe during pregnancy; animal studies have shown harmful effects on the fetus. Consult your doctor.
Use in lactation
Use in Lactation: Lomefloxacin is probably unsafe during breastfeeding; may pass into breast milk.
Pediatric use
Contraindicated in children under 18 years.
Geriatric use
Special dose adjustment needed in elderly (based on renal function).
Renal / hepatic impairment
Renal Impairment: Dose adjustment based on creatinine clearance; adjust dose for hemodialysis patients.
