Description
Lomefloxacin is a difluorinated quinolone that inhibits bacterial DNA gyrase to prevent DNA replication, transcription, and repair.
Indications
- Bacterial conjunctivitis
- Otitis externa
- Chronic suppurative otitis media with perforated tympanic membranes
- Acute otitis media with tympanostomy tubes

Pharmacology
Lomefloxacin inhibits bacterial DNA gyrase to prevent DNA replication, transcription, and repair.
Mechanism of action
Inhibits DNA gyrase to prevent bacterial cell division and repair.
Dosage
Eye Drops (Bacterial Conjunctivitis):
- Day 1: 5 drops into conjunctival sac within 20 minutes
- Days 2-7/9: 1 drop 3 times daily
Ear Drops (Otitis Externa):
- Adult and 13 years+: 10 drops (0.5 mL) once daily, 7 days
Chronic Suppurative Otitis Media:
- Adult and 12 years+: 10 drops twice daily, 14 days
Administration
- For ophthalmic/otic use only
- Hold dropper close to the eye without touching; place drop in the lower eyelid
- Wipe off extra liquid
- Apply pressure on the corner of the eye (close to the nose) for about 1 minute after instillation
- Do not touch the dropper tip to any surface or eye (to avoid contamination)
Missed dose
If you miss a dose, take it as soon as possible. If it's almost time for your next dose, skip the missed dose and go back to your regular schedule. Do not double the dose.
Side effects
Common Side Effects:
- Transient ocular burning (4.7%)
- Transient blurred vision
- Foreign body sensation
Precautions & warnings
- Caution when driving or using machines (may cause transient blurred vision)
- Do not wear contact lenses until infection clears up
- Soft hydrophilic contact lenses: benzalkonium chloride may deposit; remove lenses 15 minutes before drops
- Use within 1 month of opening
Contraindications
- History of hypersensitivity to Lomefloxacin, other quinolones, or any components
Drug interactions
- Sucralfate, aluminum/magnesium antacids, iron supplements reduce absorption
- Cimetidine increases serum levels
- Probenecid reduces renal elimination
- Increased risk of ventricular arrhythmias with Class IA/III antiarrhythmics (potentially fatal)

Use in pregnancy
Use in Pregnancy: Lomefloxacin 0.3% Eye or Ear Prep may be unsafe during pregnancy; animal studies have shown harmful effects on the fetus. Use only if benefits outweigh risks.
Use in lactation
Use in Lactation: Lomefloxacin 0.3% Eye or Ear Prep is probably unsafe during breastfeeding; caution should be exercised.
Pediatric use
Dose established for children over 1 year; safety not established in children under 1 year.
Geriatric use
Safe for elderly; no special precautions needed.
Renal / hepatic impairment

Overdose effects
