Description
Magnesium sulfate decreases levels of acetylcholine in motor nerve terminals and acts on the myocardium by decreasing the rate of SA node impulse formation and prolonging conduction time.

Pharmacology
Magnesium sulfate decreases acetylcholine levels in motor nerve terminals and acts on the myocardium by decreasing SA node impulse formation and prolonging conduction time to control seizures.
Mechanism of action
Decreases acetylcholine in motor nerve terminals and prolongs cardiac conduction time.
Dosage
Eclampsia/Severe Pre-eclampsia:
- Total initial dose: 10-14 g of magnesium sulfate
- IV: 4 g in 5% Dextrose Injection (rate not to exceed 150 mg/min)
- IM: 4-5 g (50% undiluted) into each buttock
- Subsequent IM doses: 4-5 g into alternate buttocks every 4 hours (based on patellar reflex and respiratory function)
Administration
- For hospital use only
- IV infusion rate should not exceed 150 mg/min
- Monitor blood pressure, respiration, and patellar reflex during injection
Missed dose
Not generally applicable (hospital administered); consult doctor if needed.
Side effects
Hypermagnesemia Symptoms:
- Nausea, vomiting, flushing, thirst
- Hypotension, drowsiness, confusion
- Slurred speech, double vision
- Bradycardia, muscle weakness
- Hypocalcemia, paralytic ileus
Precautions & warnings
- Caution in renal impairment, myasthenia gravis, digitalized patients
- Monitor serum-magnesium concentrations
- Caution in pregnancy
Contraindications
- Heart block
- Severe renal impairment
- Myocardial damage
Drug interactions
- Enhances neuromuscular blockers
- Enhances digitalis glycosides
Food interactions
Caution with alcohol; consult your doctor.
Use in pregnancy
Use in Pregnancy: Category D; used under supervision in eclampsia; otherwise use with caution.

Renal / hepatic impairment

Overdose effects
Magnesium toxicity - breathing difficulty, loss of reflexes; treat with calcium gluconate and supportive care.
