Description
Miconazole 2% Oral Gel is an antifungal medication that inhibits ergosterol synthesis, damaging the fungal cell wall membrane and increasing its permeability. It allows essential constituents of fungal cells to leak out, ultimately leading to fungal cell death.

Pharmacology
Miconazole inhibits a demethylation step in ergosterol biosynthesis. Ergosterol is the main sterol in yeast and fungi. Disruption in ergosterol production creates holes in the fungal cell membrane, allowing essential constituents to leak out and causing fungal cell death.
Mechanism of action
Prevents fungal growth by destroying the fungal cell wall.
Dosage
Oral Gel Dose:
- Oropharyngeal Candidiasis: Adults and children ≥2 years: 2.5 ml (1/2 spoon) 4 times daily; Infants 4-24 months: 1.25 ml (1/4 spoon) 4 times daily
- Gastrointestinal Candidiasis: 20 mg/kg/day (in 4 divided doses); Max: 250 mg (10 ml) 4 times daily
- Continue treatment for at least 1 week after symptoms resolve
Administration
- Use after meals
- Apply a small amount to the affected area with a clean finger
- Keep gel in contact with the affected area as long as possible
- Remove dental prosthesis at night and brush with gel
Missed dose
If you miss a dose, apply it as soon as you remember; skip if it's almost time for the next dose; do not double the dose.
Side effects

- Nausea and vomiting (occasionally)
- Diarrhea with long-term use
- Allergic reactions (rare)

- For external use only (oral gel is for oral use)
- In infants and young children, ensure the gel does not obstruct the throat

Contraindications
- Hypersensitivity to components
- Liver dysfunction
Drug interactions
- Warfarin: Increased anticoagulant effect
- Oral Hypoglycemics (sulfonylureas): Increased effect
- Phenytoin: Delayed metabolism, may precipitate toxicity
- Cyclosporine: May precipitate toxicity
- Astemizole, Cisapride, Terfenadine: Risk of cardiac arrhythmia (potentially fatal)
