Description
Mometasone Furoate is a synthetic corticosteroid applied topically (nasal spray and inhaler). It depresses the formation, release and activity of endogenous inflammatory chemical mediators (e.g. kinins, histamine, liposomal enzymes and prostaglandin). It inhibits the margination and subsequent cell migration to the injury site, reverses vascular dilatation and permeability, resulting in decreased access of cells to the area of injury.

Pharmacology
Mometasone depresses the formation, release and activity of endogenous inflammatory chemical mediators. It inhibits cell margination and migration and reverses vascular dilatation and permeability, thereby reducing inflammation.
Mechanism of action
Reduces inflammation and allergic reactions by suppressing inflammatory chemical mediators.
Dosage
Adult Dose (Nasal Spray):
- Allergic rhinitis: 100 mcg (2 sprays) in each nostril once daily; may increase to 200 mcg; Maintenance: 50 mcg
- Seasonal allergy prophylaxis: Start 2-4 weeks prior to pollen season
- Nasal polyps: 100 mcg once daily; may increase to twice daily after 5-6 weeks
Inhalation (Asthma):
- Mild-moderate: 400 mcg once daily in the evening; Maintenance: 200 mcg 1-2 times
- Severe: 400 mcg twice daily; titrate to lowest effective dose once controlled
Pediatric Dose (Nasal): 2-11 years: 50 mcg (1 spray) daily; >12 years: 100 mcg
Pediatric Dose (Inhalation): 400 mcg once daily (mild-moderate); 400 mcg twice daily (severe)
Administration
- Nasal spray: Shake well before use; tilt head slightly forward and spray
- Inhaler: Shake before use; press canister while breathing in slowly
- Use at the same time each day
- Rinse mouth after inhaler use (to prevent thrush)
Missed dose
If you miss a dose, take it as soon as you remember; skip if it's almost time for next dose; do not double the dose.

Uncommon (1-10%):
- Cough (7%)

Precautions & warnings
- Caution in respiratory tract tuberculosis, untreated fungal/bacterial/viral/parasitic infections
- Caution in ocular herpes simplex, nasal septum perforation, epistaxis, cataracts, glaucoma, increased intraocular pressure
- Risk of serious chickenpox and measles in susceptible individuals; avoid use in unvaccinated persons
- Deaths from adrenal insufficiency after abrupt withdrawal of oral steroids; taper gradually
- May retard growth in children
- Infections (including Candida albicans) in immunocompromised patients
- Excessive use may suppress HPA axis function
- Long-term use reduces bone mineral density
Contraindications
- Hypersensitivity
- Immunosuppressed patients
- Tuberculosis
- Status asthmaticus or acute asthma episode
- Known hypersensitivity to milk proteins or any other ingredients
Drug interactions
- Increased risk of hypokalemia with amphotericin B and potassium-wasting diuretics
- Decreases hypoglycemic effects of antidiabetic drugs
- Increased serum levels with antifungals (imidazole)
- Increased risk of tendinopathies with fluoroquinolones

Use in pregnancy
Use in Pregnancy: Mometasone may be unsafe during pregnancy (Category C); animal studies show evidence of fetal harm. Use only if clearly needed.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
Use in Lactation: Mometasone is probably safe during breastfeeding; limited human data suggests no significant risk to the baby.
Pediatric use
Safety not established in children under 2 years; doses established for 2-11 years and >12 years.
Geriatric use
Elderly may require dose adjustment (based on renal function).
Renal / hepatic impairment
Renal Impairment: Caution in kidney disease; dose adjustment may be needed.
Hepatic Impairment: Caution in liver disease; dose adjustment may be needed.
Overdose effects
Overdose may cause HPA axis suppression, hypercorticism, adrenal insufficiency; reduce or discontinue dose; symptomatic treatment.
