Description
Olmesartan blocks AT1 receptors to prevent vasoconstrictor and aldosterone-secreting effects of angiotensin II. Hydrochlorothiazide is a thiazide diuretic that increases sodium and chloride excretion through renal tubular mechanisms, reducing plasma volume and increasing renin activity.

Pharmacology
Olmesartan blocks AT1 receptors to dilate blood vessels. Hydrochlorothiazide inhibits sodium and chloride reabsorption in renal tubules to produce diuretic effect, reducing plasma volume and blood pressure.
Mechanism of action
Lowers blood pressure by dilating blood vessels and removing excess water.
Dosage
Adult Dose (Oral):
- Initial: 20/12.5 mg (olmesartan/HCTZ) once daily
- Titrate at 2-4 week intervals to 40/25 mg based on blood pressure response
Administration
- Swallow tablet whole; do not chew, crush or break
- Take at the same time each day
- May be taken with or without food
Missed dose
If you miss a dose, take it as soon as you remember; skip if it's almost time for next dose; do not double the dose.
Side effects
Common Side Effects:
- Nausea

- Hyperuricemia
- Upper respiratory tract infection
- Urinary tract infection
- Chest pain
- Back pain
- Peripheral edema
- Abdominal pain, dyspepsia, gastroenteritis, diarrhea
Precautions & warnings
- Periodically measure serum electrolytes (hypokalemia, hyponatremia)
- Hyperuricemia may occur
- Caution in impaired renal function
- Can cause fetal harm in pregnancy
Contraindications
- Hypersensitivity
- Anuria
- Hypersensitivity to other sulfonamide-derived drugs
- Pregnancy and lactation
Drug interactions
- Potentiation of orthostatic hypotension with alcohol, barbiturates or narcotics
- Dosage adjustment of antidiabetic drugs may be required
- Additive effect with other antihypertensives
- Corticosteroids, ACTH
- Lithium
Food interactions
Alcohol may potentiate orthostatic hypotension.
Use in pregnancy
Use in Pregnancy: Olmesartan + Hydrochlorothiazide is highly unsafe during pregnancy; definite evidence of severe fetal harm. Do not use.
Pregnancy Category: D — always consult a doctor before taking during pregnancy.
Use in lactation
Use in Lactation: Safety not established; do not use.
Pediatric use
Safety and effectiveness not established in pediatric patients.
Geriatric use
Caution in patients 65 years+ for dose selection.
Renal / hepatic impairment
Renal Impairment: Normal regimen if CrCl >30 mL/min; not recommended if CrCl <30 mL/min (loop diuretics preferred).
Hepatic Impairment: No dose adjustment needed.
Overdose effects
Overdose - Olmesartan: Hypotension and tachycardia; Hydrochlorothiazide: Electrolyte depletion (hypokalemia, hypochloremia, dehydration); supportive treatment.
Storage conditions
Store in a cool and dry place, protect from light and moisture; keep out of reach of children.
