Description
Perindopril is an ACE inhibitor which acts by inhibiting the conversion of angiotensin I to angiotensin II, reducing the activity of the sympathetic nervous system and inhibiting enzyme kininase, which is involved in the conversion of bradykinin and other substances.

Pharmacology
Perindopril inhibits ACE, reducing angiotensin II production, resulting in decreased vasoconstriction and aldosterone secretion. It also decreases the degradation of bradykinin, which contributes to vasodilation.
Mechanism of action
Lowers blood pressure by inhibiting ACE to reduce angiotensin II and causing vasodilation.
Dosage
Adult Dose (Oral):
- Hypertension: 4-8 mg once daily or divided q12hr; max 16 mg/day
- Stable CAD: 4 mg once daily for 2 weeks, then increase to 8 mg/day
- Heart failure: 2 mg once daily initially, max 8-16 mg/day
- Elderly: Initially 2 mg once daily; may increase to max 8 mg
- Renal impairment: CrCl <15 mL/min: 2 mg on dialysis days; 15-30: 2 mg on alternate days; 30-60: 2 mg
Administration
- Take on an empty stomach
- Swallow tablet whole; do not chew, crush or break
- Take at the same time each day
Missed dose
If you miss a dose, skip it and continue with your normal schedule; do not double the dose.

- Dizziness
Precautions & warnings
- May affect driving ability; do not drive if dizziness or weakness occurs
- May cause dizziness for the first few days; rise slowly from sitting or lying position
- Taking at bedtime may help avoid daytime dizziness
- Inform doctor if cough or throat irritation persists
- Avoid potassium supplements and potassium-rich foods
- Do not use during pregnancy and lactation
Contraindications
- History of angioedema related to previous ACE inhibitor treatment
- Pregnancy (2nd/3rd trimesters)
Drug interactions
- Diuretics: Enhanced hypotensive effect
- Potassium supplements, K-sparing diuretics: Additive hyperkalaemic effect
- Lithium: May increase serum levels and toxicity
- Aspirin or other NSAIDs: Reduced antihypertensive effect; increased risk of renal impairment
- Antidiabetic agents: Increased risk of hypoglycemia
- Gold (sodium aurothiomalate): Nitritoid reactions (rare)
- Potentially fatal: Aliskiren in patients with diabetes or renal impairment (hypotension, hyperkalemia, renal function changes)
Food interactions
Alcohol with Perindopril Erbumine may cause excessive drowsiness.
Use in pregnancy
Use in Pregnancy: Unsafe during pregnancy; definite evidence of risk to the developing baby. Contraindicated in 2nd and 3rd trimesters.
Pregnancy Category: D — always consult a doctor before taking during pregnancy.
Use in lactation
Use in Lactation: Probably safe; limited human data suggests no significant risk to the baby.
Pediatric use
Safety and efficacy not established in pediatric patients.
Geriatric use
Elderly patients: Initial dose 2 mg once daily; may increase to max 8 mg.
Renal / hepatic impairment
Renal Impairment: Caution in kidney disease; CrCl <15 mL/min: 2 mg on dialysis days; 15-30: 2 mg on alternate days; 30-60: 2 mg; consult your doctor.
Hepatic Impairment: Caution in liver disease; dose adjustment may be needed; inform doctor if signs of jaundice occur.
