Description
The vaccine is prepared from purified pneumococcal capsular polysaccharide antigens derived from 23 serotypes. It stimulates active immune response against S. pneumoniae capsular antigens, producing endogenous antibodies. Not recommended in children under 2 years as safety and efficacy have not been established and antibody response may be poor.

Pharmacology
The vaccine is prepared from 23 pneumococcal capsular polysaccharide serotypes that account for approximately 90% of invasive pneumococcal disease types: 1, 2, 3, 4, 5, 6B, 7F, 8, 9N, 9V, 10A, 11A, 12F, 14, 15B, 17F, 18C, 19F, 19A, 20, 22F, 23F, 33F. It stimulates active immune response against S. pneumoniae capsular antigens.
Mechanism of action
Produces immunity by stimulating antibody production against pneumococcal antigens.
Dosage
Primary Vaccination:
- Adults and children ≥2 years: Single dose of 0.5 mL (IM or SC)
- Not recommended in children <2 years
- Should be given at least 2 weeks before elective splenectomy, chemotherapy, or immunosuppressive treatment
- Avoid during chemotherapy/radiation therapy; at least 3 months after completion
- HIV patients: Vaccinate as soon as possible after diagnosis
Revaccination:
- Single dose 0.5 mL (IM or SC)
- Revaccination within 3 years not recommended (increased risk of adverse reactions)
- Healthy adults: Not routinely revaccinated
- High-risk patients (asplenia): May be considered at 3 years
Administration
- Administered as IM or SC injection in deltoid muscle or lateral mid-thigh
- Never inject intravascularly or intradermally

Side effects
Common Side Effects (>10%):
- Injection-site pain/soreness (60%)
- Injection-site swelling/induration (20.3%)
- Headache (17.6%)
- Injection-site erythema (16.4%)
- Asthenia and fatigue (13.2%)
- Myalgia (11.9%)
Precautions & warnings
- Delay use in significant febrile illness or active infection
- Never inject intravascularly or intradermally
- Reduced antibody response in immunosuppressed patients
- Caution in pregnancy and lactation
- Do not stop required prophylactic antibiotics
- Epinephrine (adrenaline) should be available for anaphylactic reactions
Contraindications
- Allergy to any component of the vaccine
- Fever, acute infection, or chronic disease at acute stage
- Uncontrolled epilepsy or other progressive nervous system diseases
- Vaccination within previous 3 years (unless otherwise indicated)
Drug interactions
- Can be administered simultaneously with influenza vaccine (different needles and injection sites)
Food interactions
Not food-related; it is not known whether it is safe to consume alcohol.
Use in pregnancy
Use in Pregnancy: Animal studies insufficient; should not be used unless clearly necessary; consider potential risk to fetus.
Pregnancy Category: B — always consult a doctor before taking during pregnancy.
Use in lactation
Use in Lactation: Unknown if vaccine is excreted in human milk; caution required.
Pediatric use
Not recommended in children under 2 years (safety and efficacy not established; poor antibody response).
Geriatric use
Indicated for elderly (≥65 years); higher local and systemic reaction rates after revaccination.
Renal / hepatic impairment
Renal Impairment: Caution in severe renal dysfunction; consult your doctor.
