Rupatadine is a long-acting, non-sedative antihistamine used for the symptomatic treatment of seasonal and perennial allergic rhinitis and chronic idiopathic urticaria. It acts as an antagonist at both histamine H1 receptors and PAF.
Description
Rupatadine is a long-acting, non-sedative H1-receptor antagonist. It also antagonizes platelet-activating factor (PAF). Both histamine and PAF cause bronchoconstriction and increase vascular permeability. The dual mode of action provides better therapeutic effect than an isolated antihistamine.

Pharmacology
Rupatadine antagonizes both H1 receptors and PAF, reducing the effects of allergy mediators. It also inhibits mast cell degranulation and reduces cytokine release (especially TNF-α), helping to control allergic inflammation.
Mechanism of action
Antagonizes H1 receptors and PAF to relieve allergy symptoms.
Dosage
Adults and Adolescents (>12 years):
- 10 mg (1 tablet) once daily
Children (2-11 years):
- Weight ≥25 kg: 5 ml syrup once daily
- Weight 10-25 kg: 2.5 ml syrup once daily
Administration
- Take with or without food
- Swallow tablet whole; do not chew, crush, or break
- Take at a fixed time
Missed dose
If you miss a dose, take it as soon as possible; if almost time for the next dose, skip it; do not double the dose.
Side effects
Common Side Effects:
- Somnolence
- Dizziness
- Asthenia
Uncommon:
- Increased appetite, arthralgia, back pain, impaired concentration, constipation, diarrhea, dry throat, epistaxis, fever, nausea, weight increased

- Caution in patients with known QT prolongation
- Caution in uncorrected hypokalemia
- Caution in clinically significant bradycardia or acute myocardial ischemia
- Caution in elderly (≥65 years)
- Not recommended in renal or hepatic impairment
Contraindications
- Hypersensitivity to rupatadine or any component
Drug interactions
- Ketoconazole and erythromycin: CYP3A4 inhibition increases rupatadine exposure
- Statins, CNS depressants, or alcohol: Use with caution
Food interactions
- Do not take grapefruit or grapefruit juice with Rupatadine
- Alcohol consumption is unsafe (may cause excessive drowsiness)
Use in pregnancy
Use in Pregnancy: May be unsafe; animal studies show harmful effects; use only if clearly needed; consult your doctor.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
