Description
Salbutamol + Budesonide is a combination of a β2-adrenergic agonist (salbutamol) and a corticosteroid (budesonide). Salbutamol relaxes airway smooth muscles; budesonide exhibits a wide range of inhibitory activities against multiple cell types and mediators involved in inflammation.

Pharmacology
Salbutamol stimulates β2-adrenergic receptors to relax airway smooth muscles; budesonide controls inflammation by inhibiting inflammatory cell activity and reducing cytokine release. This dual action provides rapid bronchodilation and long-term inflammation control.

Dosage
Adult Dose (Inhalation):
- Recommended: Salbutamol 180 µg + Budesonide 160 µg (2 actuations) as needed
- Maximum: 6 doses (12 inhalations) in 24 hours
- Prime before first use: Release 4 sprays into the air
- Re-prime if not used for >7 days: Release 2 sprays
Administration
- Inhale through the mouth while actuating the inhaler
- Rinse mouth after use
- Shake well before each use

Side effects
Side Effects:
- Asthma deterioration
- Paradoxical bronchospasm
- Cardiovascular effects (increased heart rate, blood pressure)
- Oropharyngeal candidiasis

- Glaucoma and cataracts
Precautions & warnings
- Paradoxical bronchospasm may occur (life-threatening); discontinue if occurs
- Monitor cardiovascular effects
- Excessive use may cause fatal cardiovascular effects
- Caution in infection-prone patients (chickenpox, measles)
- May decrease BMD (long-term use)
- May reduce growth velocity in children (indicated for ≥18 years)
Contraindications
- Hypersensitivity to salbutamol, budesonide, or any excipient
Drug interactions
- Strong CYP3A4 inhibitors (ritonavir, ketoconazole): Increased budesonide exposure; caution required
- Beta-blockers: Decreased efficacy and severe bronchospasm
- Diuretics: May potentiate hypokalemia
- Digoxin: May decrease serum digoxin levels
- MAOIs and TCAs: Use with extreme caution
