গনোরিয়া, তীব্র গনোরিয়া ইউরেথ্রাইটিস, সার্ভিসাইটিস এবং প্রোক্টাইটিস ইনট্রামাসকুলার গনোরিয়া প্রাপ্তবয়স্ক: একক ডোজ হিসাবে 2 গ্রাম। যেসব ক্ষেত্রে অ্যান্টিবায়োটিক রেজিস্ট্যান্স প্রচলিত আছে, সেক্ষেত্রে 4 গ্রাম (10 মিলি) দেওয়া যেতে পারে কিন্তু দুটি 5 মিলি ইঞ্জে, অ্যাডমিন 2টি ভিন্ন ইনজ সাইটে বিভক্ত। ইন্ট্রামাসকুলার গনোরিয়া শিশু: নবজাতকের সময়ের বাইরে এবং <45 কেজি: একক ডোজ হিসাবে 40 মিলিগ্রাম/কেজি। সর্বোচ্চ: 2 গ্রাম। অতি সংবেদনশীলতা। স্পেকটিনোমাইসিন ব্যাকটেরিয়া রাইবোসোমের 30S সাবইউনিটের সাথে আবদ্ধ হয়, এইভাবে প্রোটিন সংশ্লেষণকে বাধা দেয়। বিস্তৃত gm+ve এবং gm-ve জীবের বিরুদ্ধে এর পরিমিত কার্যকলাপ রয়েছে, যদিও অ্যানেরোবিক জীবগুলি বেশিরভাগই প্রতিরোধী। সিফিলিসের চিকিৎসায় অকার্যকর এবং ইনকিউবেটিং সিফিলিসের লক্ষণগুলিকে মুখোশ বা বিলম্বিত করতে পারে। গর্ভাবস্থা এবং স্তন্যদান। পর্যবেক্ষণের পরামিতি গনোরিয়া নির্ণয়ের সময় এবং 3 মাস পরে সিফিলিসের জন্য সেরোলজিক পরীক্ষা করুন। বমি বমি ভাব, মাথা ঘোরা, জ্বর এবং ঠাণ্ডা, অনিদ্রা, ছত্রাক, IM ইনজের পরে হালকা থেকে মাঝারি ব্যথা, কিডনি এবং লিভারের কার্যকারিতায় পরিবর্তন, Hb এবং হেমাটোক্রিট হ্রাস। সম্ভাব্য মারাত্মক: অ্যানাফিল্যাক্সিস। একযোগে ব্যবহার করলে লিথিয়াম বিষাক্ততা হতে পারে। অ্যাট্রাকিউরিয়ামের নিউরোমাসকুলার ব্লকিং প্রভাব বৃদ্ধি বা দীর্ঘায়িত করতে পারে।

Mechanism of action
Spectinomycin binds to 30S subunit of the bacterial ribosome, thus inhibiting protein synthesis. It has modest activity against a wide range of gm+ve and gm-ve organisms, though anaerobic organisms are mostly resistant.
Dosage
Adult Dose: Intramuscular Gonorrhoea Adult: 2 g as a single dose. In cases whereby antibiotic resistance is prevalent, 4 g (10 mL) may be given but divided as two 5 mL inj, admin to 2 different inj sites. Child Dose: Intramuscular Gonorrhoea Child: Beyond newborn period and <45 kg: 40 mg/kg as a single dose. Max: 2 g.
Side effects
Nausea, dizziness, fever and chills, insomnia, urticaria, mild to moderate pain after IM inj, alterations in kidney and liver function, decrease in Hb and haematocrit. Potentially Fatal: Anaphylaxis.
Precautions & warnings
Ineffective in the treatment of syphilis and may mask or delay symptoms of incubating syphilis. Pregnancy and lactation. Monitoring Parameters Perform serologic test for syphilis at the time of gonorrhoea diagnosis and 3 mth later.
Contraindications
Hypersensitivity.
Drug interactions
May result in lithium toxicity when used concurrently. May increase or prolong the neuromuscular blocking effect of atracurium.
