Description

Sucroferric Oxyhydroxide is a phosphate binder that binds to dietary phosphate through ligand exchange in the gastrointestinal tract. The bound phosphate is eliminated with feces, reducing phosphate absorption and lowering serum phosphorus and calcium-phosphorus product levels.

Pharmacology

Sucroferric Oxyhydroxide binds to phosphate in the aqueous environment of the GI tract through ligand exchange between hydroxyl groups and/or water and dietary phosphate. The bound phosphate is eliminated with feces, reducing dietary phosphate absorption and lowering serum phosphorus and calcium-phosphorus product levels.

Mechanism of action

Binds to dietary phosphate to inhibit phosphate absorption.

Dosage

Adult Dose (Oral):

  • Starting: 3 tablets (1500 mg) per day (1 tablet with each meal)
  • Dose titration: Adjust by 500 mg (1 tablet) per day based on serum phosphorus levels
  • Maximum: 6 tablets (3000 mg) per day
Administration
  • Tablets must be chewed or crushed; do not swallow whole
  • Take with meals (distribute among meals)
  • Take at the scheduled time
Missed dose

If a dose is missed, resume with the next meal; do not attempt to replace the missed dose.

Side effects

Common Side Effects:

  • Discolored feces (12%)
  • Diarrhea (6%)
Precautions & warnings
  • Monitor patients with peritonitis, significant gastric/hepatic disorders, major GI surgery history, hemochromatosis, or iron accumulation diseases
  • Caution in pregnancy and lactation
Contraindications
  • Hypersensitivity to any component
Drug interactions
  • Calcitriol, ciprofloxacin, digoxin, enalapril, furosemide, statins, hydrochlorothiazide, losartan, metoprolol, nifedipine, omeprazole, quinidine, warfarin: Can be coadministered
  • Acetylsalicylic acid, cephalexin, doxycycline: Take at least 1 hour before
  • Levothyroxine: Take at least 4 hours before

Use in pregnancy

Use in Pregnancy: No adequate controlled studies; animal studies showed post-implantation loss; use only if clearly needed; consult your doctor.

Use in Lactation: Iron absorption is minimal; excretion in breast milk is unlikely; caution required; consult your doctor.
