Description
Tigecycline is a glycylcycline antibiotic that binds to the 30S ribosomal subunit, blocking amino-acyl tRNA entry into the A site, inhibiting protein synthesis and peptide chain elongation. It is not affected by tetracycline resistance mechanisms (ribosomal protection and efflux) or by beta-lactamases, macrolide efflux pumps, and target site changes. It is generally bacteriostatic.

Pharmacology
Tigecycline binds to the 30S ribosomal subunit, blocking amino-acyl tRNA entry into the A site, inhibiting protein synthesis and peptide chain elongation. It is not affected by tetracycline resistance mechanisms and is generally bacteriostatic.
Mechanism of action
Inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit.
Dosage
Adult Dose (IV):
- Loading dose: 100 mg
- Maintenance: 50 mg every 12 hours
- IV infusion over 30-60 minutes
- Treatment duration: Complicated skin/IAA infections: 5-14 days; CAP: 7-14 days
Hepatic Impairment:
- Mild-moderate: No dose adjustment needed
- Severe: 100 mg single dose, then 25 mg every 12 hours

- Administered as IV infusion only
- Administered by a healthcare professional in a hospital
- Do not self-administer

Side effects
Common Side Effects:
- Nausea, vomiting (first 1-2 days)
- Headache
- Increased liver enzymes
- Abdominal pain, diarrhea
Less Common:
- Injection site inflammation and pain
- Allergic reactions, chills
- Thrombophlebitis, bradycardia, tachycardia
- Hypoglycemia, hyponatremia
Precautions & warnings
- Caution in hypersensitivity to tetracycline-class antibiotics
- Do not use in hospital-acquired pneumonia and ventilator-associated pneumonia
- Do not use during tooth development
- May affect driving ability (dizziness)
Contraindications
- Hypersensitivity to tigecycline or any component
- Hospital-acquired pneumonia and ventilator-associated pneumonia
- During tooth development (children and pregnant women)
Drug interactions
- Warfarin: Increased serum levels
- Oral contraceptives: Decreased efficacy
Food interactions
Alcohol is safe with Tigecycline.
Use in pregnancy
Use in Pregnancy: Unsafe; evidence of fetal risk; use only in life-threatening situations; consult your doctor.
Pregnancy Category: D — always consult a doctor before taking during pregnancy.
Use in lactation
Use in Lactation: Probably unsafe; limited data suggests drug may pass into breast milk and harm the baby; consult your doctor.
Pediatric use
Safety and efficacy not established in children <18 years
Geriatric use
No differences in safety or effectiveness between elderly and younger subjects
Renal / hepatic impairment
Renal Impairment: Safe in kidney disease; no dose adjustment needed; consult your doctor.
Hepatic Impairment: Caution in severe liver disease; dose adjustment may be needed; consult your doctor.
Overdose effects
Single IV dose of 300 mg increased nausea and vomiting; no specific antidote; provide symptomatic treatment.
Storage conditions
Before reconstitution: Store at 2-8°C in a refrigerator; do not freeze; after reconstitution: store at room temperature for up to 24 hours or at 2-8°C for up to 48 hours.
