Tropicamide + Phenylephrine + Lidocaine is indicated for cataract surgery to obtain mydriasis and intraocular anesthesia during the surgical procedure.
Description
This combination contains Tropicamide (antimuscarinic), Phenylephrine (α-adrenergic agonist), and Lidocaine (local anesthetic). Tropicamide and phenylephrine produce mydriasis, while lidocaine provides intraocular anesthesia.

Pharmacology
Tropicamide is an antimuscarinic that relaxes the sphincter muscle of the iris, causing pupillary dilatation. Phenylephrine is an α-adrenergic agonist that stimulates the dilator muscle, causing pupillary dilatation. Lidocaine blocks sodium channels, providing local anesthesia.
Mechanism of action
Produces mydriasis (tropicamide + phenylephrine) and intraocular anesthesia (lidocaine).
Dosage
Adult Dose (Intracameral):
- For cataract surgery: 0.2 ml as a single injection via intracameral route through the side port or principal port
- Instill 1-2 drops of anesthetic eye drops 5 minutes before surgery
Administration
- Administered only by an ophthalmic surgeon
- Under local anesthesia, at the beginning of cataract surgery
- Open under aseptic conditions only
- Use immediately after opening; do not reuse for the other eye or any other patient

Side effects
Common Side Effects:
- Injury to the lens (posterior capsule rupture)
- Swelling of the retina (cystoid macular edema)
- Headache
- Swelling of the cornea (keratitis)
- Increased pressure in the eye
- Redness of the eye (ocular hyperaemia)
- High blood pressure (hypertension)
Precautions & warnings
- Do not use if blister is damaged or broken
- Open under aseptic conditions only
- Use immediately after opening
- Not recommended in: combined cataract surgery with vitrectomy, shallow anterior chamber, history of acute narrow angle glaucoma
- Doctor should consider patient's medical condition if suffering from hypertension, atherosclerosis, heart disease, hyperthyroidism, prostate gland disorders, epilepsy, liver or kidney problem, breathing problem or myasthenia gravis

Storage conditions
Do not store above 30°C. Does not require refrigeration. Keep away from light and out of reach of children. For single eye use only. Use immediately after first opening. Discard remaining solution appropriately.
