Description
Vonoprazan is a potassium competitive acid blocker (P-CAB) and does not require activation by acid. It inhibits H+, K+-ATPase in a reversible and potassium-competitive manner.

- Prevention of recurrence of gastric or duodenal ulcer during low-dose aspirin administration
- Prevention of recurrence of gastric or duodenal ulcer during NSAID administration
- Adjunct to Helicobacter pylori eradication

Pharmacology
Vonoprazan is a potassium competitive acid blocker (P-CAB) and does not require activation by acid. It inhibits H+, K+-ATPase in a reversible and potassium-competitive manner.
Mechanism of action
Inhibits H+, K+-ATPase in gastric parietal cells, blocking gastric acid production.
Dosage
Adult Dose (Oral):
- Gastric ulcer: 20 mg once daily for 8 weeks
- Duodenal ulcer: 20 mg once daily for 6 weeks
- Reflux esophagitis: 20 mg once daily for 4 weeks; may extend up to 8 weeks
- Maintenance of reflux esophagitis: 10 mg once daily; may increase to 20 mg once daily
- Prevention of ulcer recurrence during low-dose aspirin or NSAID: 10 mg once daily
- H. pylori eradication: 20 mg + amoxicillin 750 mg + clarithromycin 200 mg twice daily for 7 days
Administration
- Can be taken without regard to food or timing of food
- Swallow tablets whole
Missed dose
If you miss a dose, take it as soon as you remember; if almost time for next dose, skip it; do not double the dose.
Side effects
Common Side Effects:
- Diarrhea
- Constipation
- Drug hypersensitivity (including anaphylactic shock)
- Drug eruption
- Urticaria
- Hepatotoxicity
- Jaundice
- Rash
- Nausea
- Abdominal distension
- Abnormal liver function tests
- Edema
- Eosinophilia
Precautions & warnings
- The course of the disease should closely be observed and the minimum therapeutic necessity should be used
- In long-term treatment, close observation by such means as endoscopy should be made
- In maintenance of healing of reflux esophagitis, administer only to patients who repeat recurrence and recrudescence
- Unknown if alters ability to drive; caution advised
Contraindications
- Hypersensitivity to Vonoprazan or to any excipient of the product
- Patients receiving atazanavir sulphate, nelfinavir or rilpivirine hydrochloride
Drug interactions
- Strong or moderate CYP3A inducers: drug-drug interaction may occur
Food interactions
Caution is advised when consuming alcohol with Vonoprazan.
Use in pregnancy
Use in Pregnancy: Use only if the expected therapeutic benefit is thought to outweigh any possible risk.
Pregnancy Category: C — always consult a doctor before taking during pregnancy.
Use in lactation
Use in Lactation: Avoid administration to nursing mothers; when indispensable, nursing should be discontinued.
Pediatric use
Not recommended in children and adolescents below 18 years of age
Geriatric use
Should be carefully administered in elderly patients
Renal / hepatic impairment
Renal Impairment: Mild to moderate: no dose adjustment; severe (eGFR <30 mL/min): avoid
Hepatic Impairment: Mild: no dose adjustment; moderate to severe: avoid
Overdose effects
Symptoms: No experience of overdose; not removed by hemodialysis.
Treatment: Symptomatic and supportive.

Store below 30°C, in a cool and dry place. Keep away from light. Keep all the medicine out of the reach of children.
