Baloxavir marboxil is an antiviral medication used for the treatment of influenza (flu) virus. It inhibits viral replication by blocking the cap-dependent endonuclease activity of the viral polymerase.
Indications
- Acute uncomplicated influenza in patients 12 years and older who have been symptomatic for no more than 48 hours
- Otherwise healthy patients or those at high risk of developing influenza-related complications
- Post-exposure prophylaxis (PEP) of influenza in patients 12 years and older following contact with an individual who has influenza
Dosage
Adults and Adolescents (12 years and older):
- Weight 40 to <80 kg: 40 mg (two 20 mg tablets) as a single dose
- Weight ≥80 kg: 80 mg (two 40 mg tablets) as a single dose
Hepatic Impairment: Moderate (Child-Pugh B) to normal: No clinically meaningful pharmacokinetic differences observed; Severe: Not studied.
Renal Impairment (CrCl >50 mL/min): No clinically meaningful effect identified; Severe: Not studied.

Rare: Anaphylaxis, angioedema, urticaria, erythema multiforme
Precautions & warnings
- No evidence of efficacy in illnesses caused by pathogens other than influenza viruses
- Serious bacterial infections may begin with influenza-like symptoms or may occur as a complication of influenza
- If allergic reactions (anaphylaxis, angioedema) occur, initiate appropriate treatment
- Monitor for secondary bacterial infections and treat appropriately
