Ciprofibrate is a fibrate class medication used as an adjunct to diet, exercise and weight reduction for the treatment of severe hypertriglyceridemia and mixed hyperlipidemia.

Indications
  • Severe hypertriglyceridemia (with or without low HDL cholesterol)
  • Mixed hyperlipidemia (when a statin is contraindicated or not tolerated)
Dosage

Adult Dose: 100 mg once daily (1 tablet per day). This dose should not be exceeded.

Renal Dose: Moderate renal impairment (CrCl 30-80 ml/min/1.73 m²): 1 tablet (100 mg) every other day. Severe renal impairment (CrCl <30 ml/min/1.73 m²): Should not be used.

Hepatic Dose: Use with caution in impaired hepatic function. Discontinue if AST and ALT levels rise to more than 3 times the upper limit of normal or if cholestatic liver injury is evidenced.

Elderly: As for adults but precautions should be taken for age more than 70 years.

Serious Side Effects: Myopathy, rhabdomyolysis, myoglobinuria, elevated liver enzymes.

Precautions & warnings
  • Myalgia/myopathy: Report unexplained muscle pain, tenderness or weakness immediately.
  • Hepatic function tests: Recommended every 3 months for the first 12 months of treatment.
  • Patients with rare hereditary problems of galactose intolerance, lactose deficiency or glucose-galactose malabsorption should not take this medicine.
  • Contraindicated during pregnancy and lactation.
