Amphotericin B is a systemic polyene antifungal medication used to treat severe fungal infections and visceral leishmaniasis.
Description
Fungistatic or fungicidal agent binding to ergosterol in fungal cell membranes, causing cell lysis.
Indications
Empirical therapy in febrile neutropenic patients, cryptococcal meningitis, refractory fungal infections, and visceral leishmaniasis.

Pharmacology
Binds irreversibly to ergosterol in fungal cell membranes, disrupting membrane integrity and causing cell death.
Mechanism of action
Creates transmembrane channels leading to leakage of intracellular components and cell death.
Dosage
Test dose: 1 mg IV over 20-30 min. Load: 0.25-0.5 mg/kg IV. Maintenance: 0.25-1 mg/kg IV daily or up to 1.5 mg/kg every other day.
Administration
Administered by medically trained personnel. Reconstituted with 12 mL Sterile Water for Injection per vial to yield 4 mg/mL suspension.
Missed dose
Administered in clinical settings as scheduled.
Side effects
Fever, chills, convulsions, nausea, vomiting, diarrhea, hypotension, hypertension, arrhythmias, renal toxicity.
Precautions & warnings
Must be administered by medically trained personnel under close clinical observation.
Contraindications
Known hypersensitivity to amphotericin B components.
Drug interactions
Increased toxicity with flucytosine and nephrotoxic drugs. Antagonizes azole antifungals.
